VACCINE HESITANCY PROGRAM. THE HEALTH COUNCIL WAS CONTRACTED TO PROVIDE ASSISTANCE ACROSS THE REGION TO ASSIST WITH VACCINE HESITANCY.
THE HEALTH COUNCIL CONDUCTS ONGOING NEEDS ASSESSMENTS IN ITS FOUR SERVICE COUNTIES TO IDENTIFY ISSUES IMPACTING POPULATION HEALTH. REPRESENTATIVES SIT ON HEALTH PLANNING BOARDS IN EACH COUNTY, WITH THE HEALTH COUNCIL PROVIDING TECHNICAL ASSISTANCE AND PREPARING ANALYSIS SUPPORT TO IDENTIFY VULNERABLE NEIGHBORHOODS AND POTENTIAL SOLUTIONS. CERTIFICATE OF NEED HEARINGS AND SITE APPROVALS ASSIST THE AGENCY FOR HEALTH ADMINISTRATION IN APPROVING SERVICE EXPANSION WHERE NEEDED.
HOME PARAMEDICINE PROGRAM SENDS FLORIDA-LICENSED PARAMEDICS INTO THE HOMES OF RESIDENTS WITH CHRONIC HEALTH CONDITIONS AT THE REQUEST AND DIRECTION OF THEIR PHYSICIANS. HOME ASSESSMENTS REDUCE UNNECESSARY EMERGENCY ROOM VISITS AND HOSPITALIZATIONS.
HEALTHLINK, THE SEMINOLE COUNTY SYSTEM OF CARE AND OTHER HEALTH CARE RELATED PROGRAMS. FOR THE HEALTHLINK PROGRAM THE HEALTH COUNCIL IDENTIFIES HEALTH ISSUES IN THE UNINSURED AND UNDERINSURED POPULATION, CONNECTS INDIVIDUALS WITH PRIMARY CARE HEALTH SERVICES, PROVIDES PHARMACY VOUCHERS FOR MEDICATIONS, AND ENROLLS THEM IN PRESCRIPTION ASSISTANCE PROGRAMS. INDIVIDUALS RECEIVE HEALTH SELF-CARE EDUCATION AND ARE CONNECTED WITH AN INFORMATION AND REFERRAL SERVICE FOR NON-CLINICAL NEEDS.
