SUPPORTIVE HOUSING - ENSURES CHRONICALLY HOMELESS INDIVIDUALS WITH DISABLING CONDITIONS ARE CONNECTED TO STABLE HOUSING AND LONG-TERM SUPPORTIVE SERVICES.
