THE COMMUNITY SERVICES PROGRAM PROVIDES INDEPENDENT LIVING SERVICES INCLUDING ADVOCACY, INFORMATION AND REFERRAL, PEER SUPPORT,INDEPENDENT LIVING SKILLS TRAINING, NURSING HOME TRANSITION, NURSING HOME DIVERSION, EMPLOYMENT, AND WORK INCENTIVES COUNSELING. SERVICES WERE PROVIDED IN JEFFERSON, SHELBY, BLOUNT, WALKER, ST CLAIR, AND OTHER COUNTIES IN ALABAMA. CONSUMERS ACHIEVED GOALS IN AREAS SUCH AS COMMUNITY BASED LIVING, MOBILITY/TRANSPORTATION, SELF-CARE, INFORMATION ACCESS/TECHNOLOGY, EDUCATION, VOCATIONAL, AND PERSONAL RESOURCE MANAGEMENT.


THE HOME MODIFICATION PROGRAM IS FOCUSED ON HOME DESIGN AND MODIFICATION FOR PEOPLE WITH DISABILITIES.
