TO PROMOTE AND PROVIDE COMPASSIONATE END-OF-LIFE CARE, MEETING THE MEDICAL, EMOTIONAL, AND SPIRITUAL NEEDS OF INDIVIDUALS AND THEIR FAMILIES.

DURING 2015, WE PROVIDED EXPERT AND COMPASSIONATE HOSPICE HOME CARE FOR 350 INDIVIDUALS AND THEIR FAMILIES WHO WERE LIVING WITH LIFE-LIMITING ILLNESS. AT OUR HOSPICE INPATIENT UNIT (SMITH PHAYER HOSPICE HOUSE), WE CARED FOR 210 HOSPICE PATIENTS AND THEIR FAMILIES. NOT ONLY DID WE PROVIDE THE BASIC CARE REIMBURSED BY MEDICARE, MEDICAID AND MOST PRIVATE HEALTH INSURANCE COMPANIES, WE ALSO WENT ABOVE AND BEYOND TO MAKE SURE THAT EACH PERSON RECEIVED PERSONALIZED CARE. OUR 92 VOLUNTEERS ASSISTED STAFF IN PROVIDING MUSIC, PETS, HOUSE AND YARD ASSISTANCE, HOME-COOKED MEALS, AND OTHER QUALITY-OF-LIFE COMFORTS. OUR SERVICE TO THE COMMUNITY WAS EXTENDED THROUGH THE PROVISION OF MATERIALS, PRESENTATIONS, & CONVERSATIONS THAT HELP FAMILIES MAKE INFORMED DECISIONS SUCH AS ADVANCE CARE AND NURSING HOME PLACEMENT.
OUR PALLIATIVE CARE CONSULTS PROGRAM HELPED 209 INDIVIDUALS WHOSE LIVES AND FAMILIES ARE AFFECTED BY A SERIOUS BUT NOT TERMINAL ILLNESS. OUR PALLIATIVE CARE TEAM PROVIDED ASSISTANCE WITH GOOD PAIN AND SYMPTOM MANAGEMENT, AS WELL AS HELPED THE FAMILY WITH DECISIONS ABOUT TREATMENT OPTIONS, ADVANCE CARE PLANNING, AND SECURING RESOURCES THAT MADE LIFE EASIER.
THE COUNSELORS IN OUR BEREAVEMENT CARE PROGRAM PROVIDED DIVERSE SUPPORT TO 721 INDIVIDUALS, IN ADDITION TO HOSTING SUPPORT GROUPS AND EDUCATIONAL EVENTS. GRIEF SUPPORT WAS DELIVERED IN MANY FORMS AT LOCAL SCHOOLS, IN INDIVIDUALS' HOMES AND NURSING CARE FACILITIES, IN HOSPITAL ROOMS, AND AT THE SMITH PHAYER HOSPICE HOUSE.
