VECINOS PROVIDES MEDICAL CARE TO FARMWORKERS AND FAMILY MEMBERS IN THE 8 WESTERN COUNTIES OF NORTH CAROLINA. OUTREACH WORKERS IDENTIFY MIGRANT AND SEASONAL FARMWORKERS AND COMPLETE INITIAL HEALTH ASSESSMENTS AND HEALTH EDUCATION. REGISTERED NURSES, NURSE PRACTITIONERS, AND MEDICAL DOCTORS WORK CLOSELY WITH THE OUTREACH WORKERS TO ENSURE HOLISTIC CARE AND CASE MANAGEMENT. MEDICAL PROVIDERS JOIN OUTREACH WORKERS ON A MOBILE MEDICAL CLINIC, WHICH SERVES MIGRANT FARMWORKERS DIRECTLY IN THEIR HOMES AFTER THE WORK DAY, BREAKING DOWN BARRIERS OF LANGUAGE, TRANSPORTATION, TIMING, AND COST. ADDITIONALLY, PROVIDERS AND OUTREACH WORKERS OFFER SIMILAR SERVICES WEEKLY IN A TRADITIONAL, STATIC CLINIC SETTING FOR SEASONAL FARMWORKERS AND FAMILY MEMBERS. HEALTH EDUCATION COMPLEMENTS THE MEDICAL CARE PROVIDERS OFFER.
