WESLEY HOMES COMMUNITY HEALTH SERVICES PROVIDES HOURLY AND LIVE-IN CARE TO CLIENTS IN THEIR OWN HOMES. HOME HEALTH AIDE, COMPANION CARE, AND HOMEMAKING SERVICES ARE PROVIDED TO CLIENTS IN NEED OF SOCIAL AND PERSONAL CARE. THOSE CLIENTS WITH MORE ADVANCED HEALTH CARE NEEDS RECEIVE THE SERVICES OF REGISTERED NURSES OR LICENSED PRACTICAL NURSES. THE ORGANIZATION EDUCATES THE PUBLIC ON THE VALUE OF HOME HEALTH SERVICES AND ASSISTS CAREGIVERS THROUGH EDUCATION OF CAREGIVING SKILLS, KNOWLEDGE, AND RESOURCES TO IMPROVE THE CARE FOR THE PATIENT AND ALLEVIATE THE STRESS OF THE CAREGIVER. THE TEAM ADDRESSES PAIN AND SYMPTOM MANAGEMENT, BEREAVEMENT SERVICES, AND SPIRITUAL COUNSELING OF THE PATIENT AND LOVED ONES.
