MEDICAL CARE: CARETEAM PLUS FAMILY HEALTH AND SPECIALTY CARE (CARETEAM+) PROVIDES MEDICAL CARE SERVICES TO ADULT AND PEDIATRIC PATIENTS IN THE CONWAY/MYRTLE BEACH AREA, SPECIALIZING IN CHRONIC DISEASE MANAGEMENT, ASTHMA, DIABETES, HIGH CHOLESTEROL, HYPERTENSION, OBESITY, HIV, HEPATITIS AND SMOKING CESSATION. CARETEAM+ PROVIDES ONSITE PHLEBOTOMY, COUNSELING, CASE MANAGEMENT, TEEN SERVICES AND TELEMEDICINE SERVICES. CARETEAM+ HAS FOR OVER 29 YEARS PROVIDED HIV TREATMENT, PREVENTION AND SOCIAL SERVICES TO PEOPLE LIVING WITH OR AT RISK FOR HIV INFECTION IN HORRY, GEORGETOWN AND WILLIAMSBURG COUNTIES. THESE SERVICES INCLUDE MEDICAL CARE, MEDICAL CASE MANAGEMENT, DENTAL, SUBSTANCE ABUSE AND MENTAL HEALTH COUNSELING, AND PRE- AND POST EXPOSURE PROPHYLACTIC TREATMENT FOR PREVENTION OF HIV INFECTION. CARETEAM+ PROVIDES MEDICAL SERVICES REGARDLESS OF ABILITY TO PAY OFFERING A SLIDING FEE SCALE FOR THE UN- AND UNDERINSURED PATIENTS. CARETEAM+ ALSO PROVIDES HIV PATIENTS WITH INSURANCE PREMIUM ASSISTANCE.
HOUSING ASSISTANCE FOR HIV+: CARETEAM+ PROVIDES EMERGENCY ASSISTANCE WITH HOUSING INCLUDING FINANCIAL SUPPORT FOR RENT/UTILITIES AND MORTGAGE FOR THOSE WHO QUALIFY FOR HOUSING OPPORTUNITIES FOR PEOPLE WITH AIDS PROGRAM.
HIV/STI PREVENTION COUNSELING, EDUCATION AND TESTING SERVICES: CARETEAM+ PROVIDES ONSITE RAPID TESTING FOR HIV, HEP C AND OTHER STIS. NEW SERVICE INCLUDE PRE-EXPOSURE PROPHYLACTIC TREATMENT FOR PATIENTS WHO ARE AT HIGH RISK FOR HIV INFECTION INCLUDING NEGATIVE PARTNERS OF THOSE LIVING WITH HIV.
