REVIEWS AND COUNSELS PARTICIPANTS ON SCREENING RESULTS 4. PROVIDES PARTICIPANTS WITH IDENTIFIED COMMUNITY MEDICAL RESOURCES FOR INTERVENTION TREATMENT 5. PROVIDES OUTCOME SCREENING DATA TO HOSPITALS THAT INCLUDE HIGH RISK PARTICIPANT SCREENING RESULTS AND CONTACT INFORMATION FOR HOSPITALS TO FOLLOW-UPBEYOND THE HOSPITAL PROGRAM: 1.
