STROKE RISK IDENTIFICATION PROGRAM: 1. PROVIDES EVIDENCED-BASED AND STANDARDIZED SCREENING PROTOCOL FOR COMMUNITY STROKE RISK SCREENINGS 2. IDENTIFIES INDIVIDUALS WHO ARE AT HIGH RISK FOR STROKE 3. REVIEWS AND COUNSELS PARTICIPANTS ON SCREENING RESULTS 4. PROVIDES PARTICIPANTS WITH IDENTIFIED COMMUNITY MEDICAL RESOURCES FOR INTERVENTION TREATMENT 5. PROVIDES OUTCOME SCREENING DATA TO HOSPITALS THAT INCLUDE HIGH RISK PARTICIPANT SCREENING RESULTS AND CONTACT INFORMATION FOR HOSPITALS TO FOLLOW-UPBEYOND THE HOSPITAL PROGRAM: 1.
