PROVIDE PREVENTIVE SERVICES AND HEALTHY LIFESTYLE EDUCATION TO THE COMMUNITY TO COMBAT HEART DISEASE, DIABETES AND HYPERTENSION. IN 2021, SERVICES AND EDUCATION WERE PROVIDED TO 7638 AREA RESIDENTS THROUGH OUR NEW BEGINNINGS EDUCATION PROGRAM, COMMUNITY ACCESS PROGRAM, AND OUR COMPREHENSIVE CHRONIC CARE FREE CLINIC FOR UNDER-INSURED AND UNINSURED. OVER 16,160 POUNDS OF FRESH FRUIT AND VEGETABLES WERE DITRIBUTED ALONG WITH 223 FARMERS TO FAMILIES FOOD BOXES CHILDHOOD WELLNESS PROGRAMMING FOR FAMILIES OF 496 CHILDREN. SENIOR PROGRAMMING FOR 1,172 RESIDENTS. FREE CLINIC OUTCOMES CONTINUE TO EXCEED NATIONAL AVERAGES.
