EMERGENCY SERVICES TO PREVENT AND END HOMELESSESS: FACETS SUPPORTS FAMILIES AND INDIVIDUALS WHO ARE HOMELESS OR AT RISK OF BECOMING HOMELESS BY OFFERING BASIC NEEDS ASSISTANCE, PREVENTION SERVICES AND COMPREHENSIVE CASE MANAGEMENT. FACETS ALSO OFFERS HOT MEALS PROGRAMS, OUTREACH AND SUPPORT PROGRAMS FOR SINGLE ADULTS, MEDICAL AND DENTAL RESOURCES, AND PERMANENT HOUSING SOLUTIONS. IN ADDITION, THE HYPOTHERMIA PREVENTION PROGRAM OPERATES THROUGHOUT THE WINTER PROVIDING SHELTER, HOT MEALS AND CASE MANAGEMENT SERVICES.
