PROGRAM WHEREIN APPALACHIAN STATE AND COUNTY POLICYMAKERS AND PROVIDERS WILL HAVE BECOME MORE AWARE OF THE CHALLENGES OF APPALACHIAN SOCIAL DETERMINANTS OF HEALTH, INCLUSIVE OF CULTURAL AND STRUCTURAL BARRIERS, TO MORE PRECISELY CREATE, AND DEVELOP NEW INTERVENTIONS AND RESOURCES THAT REDUCE BARRIERS TO HEALTH CARE ACCESS AND UTILIZATION AS IT RELATES TO CANCER, HCV, HIV< SUD, AND OTHER EMERGING HEALTH CONCERNS.
