PROGRAM THAT ADDRESSES HEALTH AND MENTAL HEALTH INEQUITIES OR DISPARITIES IN HEALTH OR MENTAL HEALTH, OR THE FACTORS THAT SHAPE HEALTH, THAT ARE SYSTEMIC AND AVOIDABLE AND, THEREFORE, CONSIDERED UNJUST OR UNFAIR.


PROGRAM WHEREIN APPALACHIAN STATE AND COUNTY POLICYMAKERS AND PROVIDERS WILL HAVE BECOME MORE AWARE OF THE CHALLENGES OF APPALACHIAN SOCIAL DETERMINANTS OF HEALTH, INCLUSIVE OF CULTURAL AND STRUCTURAL BARRIERS, TO MORE PRECISELY CREATE, AND DEVELOP NEW INTERVENTIONS AND RESOURCES THAT REDUCE BARRIERS TO HEALTH CARE ACCESS AND UTILIZATION AS IT RELATES TO CANCER, HCV, HIV< SUD, AND OTHER EMERGING HEALTH CONCERNS.


PROGRAM TO SUPPORT EFFORTS TO EXPAND THE REACH AND IMPACT OF HIV SELF-TESTING (HIVST) PROGRAMS BY BUILDING THE CAPACITY OF COMMUNITY-SERVING ORGANIZATIONS (CSOS) FOCUSED ON POPULATIONS MOST AFFECTED BY HIV.
