EDUCATION AND TRAINING: HCCI IS AT THE FOREFRONT OF TRAINING PROFESSIONALS IN HOME-BASED PRIMARY CARE, DEVELOPING THE FIRST-EVER EDUCATION CURRICULUM THAT EXPLORES THE FULL SPECTRUM OF HOME-BASED PRIMARY CARE MEDICAL, SOCIAL AND ECONOMIC. OUR PROGRAM, DESIGNED FOR CLINICIANS, PRACTICE MANAGERS AND OTHER PROFESSIONALS INTERESTED IN HOME-BASED PRIMARY CARE, CONSISTS OF CLASSROOM EDUCATION, MENTORSHIP, HANDS-ON FELLOWSHIP AND ONLINE LEARNING OPPORTUNITIES.
PRACTICE ENGAGEMENT AND IMPROVEMENT: ASSISTING PRACTICES WITH ONGOING DEVELOPMENT AND EXPANSION OF HOME BASED PRIMARY CARE.
RESEARCH: TO SOLIDIFY ITS PLACE AS A UNIQUE MODEL OF CARE, IT IS CRITICAL THAT HOME-BASED PRIMARY CARE (HBPC) BE BUILT ON EVIDENCE-BASED PRACTICES. HCCI IS CONTINUING TO GATHER INSIGHTS AND DEVELOP A GREATER UNDERSTANDING OF BEST CLINICAL AND OPERATIONAL PRACTICES. IN ADDITION, HCCI WORKS WITH ITS PARTNERS TO PROVIDE ONGOING RESEARCH TO DEMONSTRATE THE BENEFITS OF HBPC AND BEST WAYS TO IMPLEMENT AND EXPAND PRACTICES.
