OUR MISSION IS TO PROVIDE ACCESS TO A COORDINATED SYSTEM OF HEALTH CARE AND COMMUNITY RESOURCES FOR THOSE THAT ARE UNDERSERVED AND IMPACTED BY COMMUNITY CONDITIONS.

THRIVE: IN 2016 OHIO HAD ONE OF THE WORST INFANT MORTALITY RATES IN THE COUNTRY, WITH STARK COUNTY HAVING ONE OF THE WORST RATES IN THE STATE AT 8.23. THE RACIAL DISPARITY RATE WAS 20.24 FOR NON-HISPANIC BLACKS AND 6.66 FOR NON-HISPANIC WHITES. THRIVE'S GOAL WAS TO USE COMMUNITY RESOURCES, HOME VISITING PROGRAMS AND EDUCATION ON SAFE SLEEP TO REDUCE THOSE NUMBERS. THRIVE WON A NOVA AWARD IN JULY 2019 FOR ITS WORK. OUR CURRENT INFANT MORTALITY RATE IS 6.0 WITH NO RACIAL DISPARITY. WE HAVE 3 TO 4 CHWS WORKING AS PART OF THE THRIVE INITIATIVE TO REDUCE INFANT MORTALITY DEPENDING ON NEED AND FUNDING. 8 CHWS FOCUSING ON HELPING CLIENTS WITH CHRONIC DISEASES ADDRESS THEIR SOCIAL DETERMINANTS OF HEALTH, OBTAIN, AND BOND WITH A PRIMARY CARE PROVIDER, AND IMPROVE THEIR HEALTH AS INDICATED BY POSITIVE TRENDS IN LAB VALUES. THE DATA OF BOTH THE THRIVE AND CHRONIC DISEASE CHWS IS DIRECTED BY THE PATHWAYS CARE COORDINATION SYSTEM AND THE PATHWAYS HUB. THE HUB SERVES AS A DATA COLLECTION SITE FOR EVALUATION OF THE EFFECTIVENESS OF CHWS FROM KENT STATE AND FOR REIMBURSEMENT FOR COMPLETION OF PATHWAYS BY THE MEDICAID MANAGED CARE ORGANIZATIONS IN OHIO.
