OUR GOAL IS TO IMPROVE THE HEALTH OUTCOMES AND STANDARD OF LIVING OF THE COMMUNITY AS A WHOLE. AS AN FQHC, WE ARE DEDICATED TO PROVIDING PRIMARY CARE, DENTAL CARE, BEHAVIORAL HEALTH, AND OUTREACH SERVICES TO THIS COMMUNITY TO THE UNDERSERVED ON A SLIDING FEE SCALE. WE SERVED 5,732 UNDUPLICATED PATIENTS IN 2021 COMPARED TO 5,475 IN 2020. WE ALSO OPENED A SECOND SITE IN MIDDLETOWN OHIO WHICH ALLOWED US TO SERVE A WIDER POPULATION AND INCREASE ACCESS FOR MORE PATIENTS. TOTAL VISITS WERE 20,280 IN 2021 COMPARED TO 17,471 IN 2020.
