THE PUBLIC HEALTH WORKER (PHW) GRANT PROVIDES INDEPENDENT CONNECTION THE OPPORTUNITY TO FOCUS ON THE PUBLIC HEALTH AND WELLBEING OF PEOPLE WITH DISABILITIES, SUPPORT HEALTHCARE PROVIDERS, AND ENGAGE OTHER COMMUNITY AND INDEPENDENT LIVING NETWORK PARTNERS. INDEPENDENT CONNECTION PROVIDES SUPPORT ON A VARIETY OF TOPICS THAT INCLUDE VACCINATION, GOAL SETTING/SKILL BUILDING, SOCIAL ACTIVITIES, MENTAL HEALTH, FOOD ACCESS, EMERGENCY PREPAREDNESS, ETC.
