TO PROVIDE QUALITY LIFE STYLES FOR PERSONS WITH A DISABILITY THROUGH THE PROVISION OF SERVICE ADMINISTRATION, INDIVIDUALIZED PLANNING, AND PAYROLL SERVICES FOR IN-HOME SUPPORT EMPLOYERS.

LIFE PATTERNS, INC. WORKS WITH APPROXIMATELY 1,100 INDIVIDUALS WITH A DISABILITY AND THEIR FAMILY MEMBERS WHO HAVE CHOSEN THE SELF-DIRECTED CARE SERVICE DELIVERY MODEL FOR MEDICAID HOME AND COMMUNITY BASED SERVICE RECIPIENTS. LIFE PATTERNS HAS DEVELOPED A TRAINING NOTEBOOK AND PROVIDES ONE-ON-ONE ASSISTANCE TO INDIVIDUALS WITH PLANNING IN IMPLEMENTING PERSONALIZED SUPPORT SYSTEMS AND ASSISTANCE WITH THE HIRING PROCESS OF PERSONAL SUPPORT PROVIDERS. LIFE PATTERNS ALSO ASSIST WHEN EMPLOYEE PERFORMANCE IS NOT ADEQUATE AND REMEDIATION IS NEEDED, BACK-UP SUPPORT ISSUES, QUESTIONS ABOUT FINDING AND NAVIGATING THE DEVELOPMENTAL AND PHYSICAL DISABILITY SYSTEM OF KANSAS. LIFE PATTERNS PROVIDES TRAINING ON THE PERSON CENTERED PLANNING PROCESS WHICH IS REQUIRED BY THE STATE OF KANSAS. THE STATE OF KANSAS OFFERS THE OPTION OF A LIMITED SCOPE LICENSE FOR SERVICES FOR A PERSON WITH A DISABILITY. LIFE PATTERNS, INC. WAS AMONG THE FIRST IN KANSAS TO OBTAIN A LIMITED LICENSE AND HAS DEVELOPED A TRAINING NOTEBOOK WITH THE EXPERIENCE GAINED BY GOING THROUGH THE APPLICATION AND IMPLEMENTATION PROCESS. WHEN REQUESTED, LIFE PATTERNS PROVIDES ASSISTANCE IN DETERMINING IF THIS IS AN OPTION THE RECIPIENT SHOULD PURSUE. LIFE PATTERNS ALSO ASSISTS LIMITED LICENSES WITH BILLING AND PAYROLL SERVICES, AS WELL AS INFORMATION AND ASSISTANCE ON FINAL RULE REQUIREMENTS AND INDUSTRY AND EMPLOYMENT RELATED ISSUES.
