THE ORGANIZATION'S MISSION IS TO STRENGTHEN UNDERSERVED COMMUNITIES, ONE PATIENT AT A TIME. THIS IS ACCOMPLISHED THROUGH COMMUNITY CLINICS PROVIDING PRIMARY CARE, PHYSICALS, IMMUNIZATIONS, ACUTE CARE, CHRONIC DESEASE MANAGEMENT, SCREENINGS AND PREVENTATIVE CARE. THESE CLINICS TARGET UNDERSERVED PATIENTS AND PROVIDE CARE AT LITTLE OR NO COST.

IN FY 2024, IFM SERVED 5,165 UNIQUE PATIENTS AND PROVIDED 10,560 PATIENT APPOINTMENTS ACROSS 31 CLINIC SITES IN COLLABORATION WITH 24 COMMUNITY PARTNERS. THIS REPRESENTS SIGNIFICANT PROGRAM GROWTH OVER THE PRIOR YEAR. WE ADDED AN ADDITIONAL COMMUNITY PARTNER CLINIC SITE IN 2024: THE GUARDIAN ANGEL SETTLEMENT ASSOCIATION'S CHILD DEVELOPMENT CENTER IN ST. LOUIS CITY. OUR TWO NEWEST COMMUNITY CLINIC SITES, THE COMMUNITY CONNECTIONS HEALTH CLINIC IN BRIDGETON, MO AND YOUTH AND FAMILY CENTER HEALTH CLINIC IN NORTH ST. LOUIS CITY, BOTH INCREASED CLINIC HOURS, PROGRAMMING, AND SUBSEQUENTLY INCREASED THE NUMBER OF PATIENTS SERVED SINCE THEY STARTED SEEING PATIENTS IN 2023. THESE TWO SITES, IN ADDITION TO OUR HANCOCK PLACE COMMUNITY CLINIC, REPRESENT OUR COMMUNITY CLINICS WHICH MODEL SERVE NOT ONLY THE CLIENTS OF THE COMMUNITY PARTNER WHERE THE CLINIC IS LOCATED, BUT ALSO SERVE INDIVIDUALS IN NEED FROM THE ENTIRE SURROUNDING COMMUNITY.AT THESE SITES, WE ALSO HAVE COMMUNITY HEALTH WORKER SUPPORT AND SERVICES THAT ADDRESS PATIENT SOCIAL DETERMINANT OF HEALTH NEEDS WHICH INCLUDE FOOD INSECURITY, HOUSING, BASIC CARE ITEMS, SUPPORT WITH UTILITIES AND TRANSPORTATION, MENTAL HEALTH, AND REFERRALS TO OTHER SUPPORTIVE SERVICES IN THE COMMUNITY. IFM ALSO WON TWO AWARDS IN THE LAST YEAR INCLUDING THE NORTH COUNTY INC.'S COMMUNITY DEVELOPMENT LEADERSHIP AWARD FOR OUR PARTNERSHIP WITH SSM DEPAUL/CONNECTIONS FOR HEALTH AND HEALING PROGRAM AND THE SPIRIT OF DELMAR DIVINE AWARD FOR OUR PARTNERSHIP WITH YOUTH AND FAMILY CENTER.
