OUR MISSION IS TO ENABLE COMMUNITY SENIORS TO LIVE HEALTHY, INDEPENDENT AND SOCIALLY CONNECTED LIVES.

OUR PRIMARY SERVICES FOR OLDER ADULTS INCLUDE HEALTH PROMOTIONAL ACTIVIIES, NURSING SERVICES, VOLUNTEER SERVICES, OUTREACH TO SOMALI ELDERS, AND FOOD DISTRIBUTION. WE ALSO PROVIDE SERVICE COORDINATION, INFORMATION AND REFERRALS, AND SUPPORT TO CAREGIVERS. HEALTH PROMOTIONAL ACTIVITIES INCLUDE SUCH THINGS AS NURSE IS IN CLINICS, SENIOR SOCIAL/HEALTH TALKS, LAUGHTER YOGA CLASSES, DIABETES SUPPORT GROUP, AND SOLOS RESOURCE GROUP. NURSING SERVICES INCLUDE NURSE VISITS, FOOTCARE, HEALTH ASSESSMENTS, AND CASE MANAGEMENT. VOLUNTEER SERVICES INCLUDE TRANSPORTATION TO MEDICAL APPOINTMENTS AND GROCERY SHOPPING, COMPANION SERVICES TO ISOLATED SENIORS, LIGHT CHORES AND HOMEMAKER SERVICES, AND DELIVERING FOOD.
