AS A MEMBER OF THE ESSENTIA HEALTH FAMILY, GRACEVILLE HEALTH CENTER'S MISSION AS A CATHOLIC, BENEDICTINE SPONSORED FACILITY IS TO PROMOTE CHRIST'S MINISTRY OF HOLISTIC HEALING FOR ALL HUMAN LIFE WITH SPECIAL CONCERN FOR THE POOR AND POWERLESS.

GRACEVILLE HEALTH CENTER, DBA ESSENTIA HEALTH HOLY TRINITY HOSPITAL, IS ORGANIZED AND OPERATED EXCLUSIVELY FOR CHARITABLE, RELIGIOUS, EDUCATIONAL, AND SCIENTIFIC PURPOSES. MORE SPECIFICALLY, ESSENTIA HEALTH HOLY TRINITY HOSPITAL IS CREATED AND ORGANIZED TO OWN, MAINTAIN, OPERATE AND CONDUCT, DIRECTLY OR INDIRECTLY, AND TO ASSIST AND COORDINATE ACTIVITIES OF FACILITIES FOR HEALTH CARE, EDUCATION, CARE FOR THE AGED AND SOCIAL SERVICES IN ACCORDANCE WITH THE CHARITABLE WORKS TRADITION OF THE ROMAN CATHOLIC CHURCH. IN KEEPING WITH THIS SPECIFIC PURPOSE, ALL WORKS SHALL BE CARRIED OUT IN ACCORDANCE WITH THE CHARISM OF THE BENEDICTINE SISTERS BENEVOLENT ASSOCIATION, A MINNESOTA NONPROFIT CORPORATION.ESSENTIA HEALTH HOLY TRINITY HOSPITAL HAS A 15-BED CRITICAL ACCESS HOSPITAL AND PRIMARY CLINICS THAT SERVE THE PEOPLE OF GRACEVILLE AND THE SURROUNDING COMMUNITIES IN BIG STONE COUNTY. THE HOSPITAL IS A LEVEL IV TRAUMA CENTER AND ACUTE STROKE READY HOSPITAL. THE EMERGENCY DEPARTMENT IS STAFFED 24 HOURS A DAY. THE CLINICS PROVIDE PRIMARY CARE SERVICES AND ACCESS TO SPECIALTY SERVICES BOTH IN PERSON AND THROUGH TELEMEDICINE. IN ADDITION, ESSENTIA HEALTH HOLY TRINITY HOSPITAL HAS A 50-BED SKILLED NURSING FACILITY AND ASSISTED LIVING APARTMENTS.ESSENTIA HEALTH HOLY TRINITY HOSPITAL EMPLOYS OVER 70 FULL TIME EQUIVALENTS. THE HOSPITAL PROVIDED APPROXIMATELY 290 HOSPITAL PATIENT DAYS AND NEARLY 9,000 OUTPATIENT VISITS DURING THE FISCAL YEAR ENDED JUNE 30, 2021. THE CLINICS HAD OVER 4,000 ENCOUNTERS AND THE SKILLED NURSING FACILITY PROVIDED OVER 7,000 RESIDENT DAYS DURING THE SAME TIME PERIOD.DURING THE FISCAL YEAR ENDED JUNE 30, 2021, ESSENTIA HEALTH HOLY TRINITY HOSPITAL PROVIDED THE FOLLOWING COMMUNITY BENEFITS: $90,000 IN CHARITY CARE, $206,000 COSTS IN EXCESS OF MEDICAID PAYMENTS, $8,000 IN COMMUNITY SERVICES AND $5,000 IN HEALTH PROFESSION EDUCATION.
