(FHC) SERVED APPROXIMATELY 55,124 UNIQUE PATIENTS FOR THE 12 MONTHS ENDING DECEMBER 31, 2021 WHO WOULD NOT OTHERWISE RECEIVE CARE OR WOULD NEED TO OVERCOME SIGNIFICANT BARRIERS TO ACCESS CARE.
