TO PROVIDE SUPPORT AND COORDINATE SERVICES TO ENABLE ADULTS WITH INTELLECTUAL AND DEVELOPMENTAL DISABILITIES TO LIVE IN THEIR OWN HOMES. THE AGENCY ASSISTS PEOPLE TO MAKE INFORMED CHOICES AND REACH THEIR OWN GOALS, WITH SUPPORT AVAILABLE TO THE EXTENT AND FOR AS LONG AS IT IS NEEDED.

OPTIONS IN COMMUNITY LIVING PROVIDES SERVICES TO INDIVIDUALS WITH INTELLECTUAL AND DEVELOPMENTAL DISABILITIES LIVING IN DANE COUNTY WHO ARE ELIGIBLE FOR STATE OF WISCONSIN LONG TERM CARE SERVICES. THE AGENCY'S OBJECTIVE IS TO CONTINUE TO MEET THE CHANGING NEEDS OF THOSE SERVED AND ASSIST INDIVIDUALS TO LIVE SUCCESSFUL AND FULL LIVES AS MEMBERS OF OUR COMMUNITY. IT IS ALSO OUR OBJECTIVE TO PREVENT FACILITY PLACEMENTS FOR THE INDIVIDUALS SERVED. IN 2021, THE AGENCY PROVIDED OVER 447,500 HOURS OF SUPPORTED LIVING SERVICES TO 111 INDIVIDUALS WITH INTELLECTUAL AND DEVELOPMENTAL DISABILITIES LIVING IN THEIR OWN HOMES AND APARTMENTS.
