ANNE'S SALVADORIAN CAMPUS OPERATES A 50-BED LONG TERM CARE FACILITY, 43-UNIT RESIDENTIAL CARE APARTMENT COMPLEX, 26-BED CBRF, AND A CONVENT SERVING RETIRED RELIGIOUS IN A SUPPORTIVE CARE ENVIRONMENT. THE HOME PROVIDES CARE TO THE INDIGENT UNDER MEDICAID PROGRAM. THE HOME ALSO FOCUSES A VARIETY OF SOCIAL ACCOUNTABILITY EFFORTS TO ENHANCE EDUCATIONAL, SOCIAL, AND SPIRITUAL BENEFITS TO A VARIETY OF CONSTITUENTS IN THE GREATER MILWAUKEE AREA. THE FOLLOWING IS A SUMMARY OF THESE SERVICES PROVIDED BY THE HOME TO SUPPORT SOCIAL ACCOUNTABILITY: - CLINICAL SITE TRAINING- PASTORAL CARE SERVICES AND OTHER SPIRITUAL SUPPORT FOR THOSE IN NEED- EDUCATION AND WELLNESS PROGRAMS FOR THE ELDERLY- SUPPORT GROUPS FOR EDUCATION ON ALZHEIMER'S AND OTHER DISEASES- VOLUNTEER COORDINATOR FOR COMMUNITY-BASED FELLOWSHIP AND FAMILY SUPPORT- SUPPORT EFFORTS FOR A VARIETY OF EMERGENCY NEEDS FOR RESIDENTS


ASSISTED LIVING CONSISTS OF A 43-UNIT RESIDENTIAL CARE APARTMENT COMPLEX AND A 26-UNIT COMMUNITY-BASED RESIDENTIAL FACILITY THAT PROVIDES 24/7 SUPPORTIVE LIVING AND ENHANCED ASSISTED LIVING OFFERED TO FRAIL ELDERS. ASSISTED LIVING PROVIDES A FULL COMPLEMENT OF ACTIVITIES, SOCIAL SERVICES, AND PASTORAL CARE AND IS CONTRACTED WITH FAMILY CARE - COMMUNITY WAVIER.


SISTERS RESIDENCE IS A CONVENT FOR RETIRED SISTERS OF THE DIVINE SAVIOR. CARE AND SERVICES ARE FUNDED BY MY CHOICE FAMILY CARE AND SDS.
