VNA HOME HEALTH IS A MEDICARE CERTIFIED HOME CARE AGENCY AND PROVIDES CARE FOR PATIENTS IN THEIR HOME IN ACCORDANCE WITH PHYSICIAN ORDERS. OUR CLINICAL STAFF WORKS DIRECTLY WITH PATIENTS, FAMILIES, AND THEIR PHYSICIANS TO ASSIST PATIENTS TO THEIR MAXIMUM LEVEL OF HEALTH AND INDEPENDENCE. AS A LONG-STANDING AGENCY IN THIS COMMUNITY, IT IS OUR GOAL TO KEEP PEOPLE SAFE AND HEALTHY IN THEIR HOMES FOR AS LONG AS POSSIBLE. DUE TO OUR AGENCY'S VAST NUMBER OF SERVICES, INCLUDING HOME HEALTH, HOSPICE, AND PRIVATE DUTY, WE CAN ENSURE OUR PATIENTS' QUALITY OF CARE IS MAINTAINED THROUGHOUT THE CONTINUUM OF CARE.
VNA BLUE WATER HOSPICE PROVIDES COMPREHENSIVE, HOLISTIC, AND END-OF-LIFE CARE. PATIENTS AND FAMILIES FACING A LIFE-LIMITING ILLNESS RECEIVE SERVICES WHICH FOCUS ON COMFORT MEASURES AND EMOTIONAL SUPPORT. HOSPICE PROVIDES PHYSICAL CARE AND EXPERTISE IN PAIN MANAGEMENT AND SYMPTOM CONTROL UNDER THE DIRECTION OF A PHYSICIAN. IN ADDITION, HOSPICE ALSO ADDRESSES THE EMOTIONAL, SOCIAL, PSYCHOLOGICAL, AND SPIRITUAL NEEDS OF THE PATIENT AND FAMILY. HOSPICE MAKES IT POSSIBLE FOR THE PATIENT AND LOVED ONES TO SPEND FINAL DAYS TOGETHER, SURROUNDED BY FAMILIAR SIGHTS AND SOUNDS. AFTER A LOVED ONE'S JOURNEY ENDS, WE OFFER ONGOING BEREAVEMENT SUPPORT FOR FAMILY AND FRIENDS. WHILE WE CAN'T CHANGE THE OUTCOME, WE CAN EASE THE JOURNEY.
VNA BLUE WATER HOSPICE HOME IS A DEDICATED FACILITY PROVIDING TERMINALLY ILL PATIENTS EXPERT COMFORT CARE AS THEIR ILLNESS PROGRESSES AND WHEN THEIR OWN HOME IS NO LONGER AN OPTION. OUR HOME IS A COMFORTABLE AND PERSONALIZED SETTING WHERE REGISTERED NURSES AND SUPPORT STAFF PROVIDE 24-HOUR CARE, AND WHERE FAMILY AND FRIENDS CAN SPEND QUALITY TIME WITH THEIR LOVED ONES THROUGH THEIR END-OF-LIFE JOURNEY.
VNA PRIVATE DUTY (SPECIAL CARE) SERVICES HELP SENIORS WITH PHYSICAL OR MENTAL LIMITATIONS, THAT COME WITH AGING, TO BE ABLE TO CONTINUE TO LIVE INDEPENDENTLY IN THEIR OWN HOME. UNDER RN SUPERVISION, OUR CERTIFIED HOME HEALTH AIDES AND BASIC CARE PROVIDERS ASSIST CLIENTS IN THE ACTIVITIES OF DAILY LIVING. ALLOWING SENIORS TO REMAIN INDEPENDENT IN THEIR HOME INCREASES THE QUALITY OF LIFE AND CREATES A BETTER ALL-AROUND SITUATION. VNA CUSTOMIZES A CARE PLAN THAT PROVIDES THE RIGHT MIX OF SERVICES TO PRESERVE A LOVED ONE'S INDEPENDENCE, MAINTAIN IN-HOME SAFETY, AND ENHANCE QUALITY OF LIFE.
