CARE COORDINATION UNIT THE CARE COORDINATION UNIT (CCU) IS A DEPARTMENT OF MANY PROGRAMS AND MANY INITIATIVES AND PILOT PROGRAMS. ALL PROGRAMS ARE DESIGNED TO ASSIST INDIVIDUALS AGE 60 PLUS REMAIN INDEPENDENT AND IN THEIR OWN HOMES AS LONG AS IT IS SAFELY POSSIBLE. PROGRAMS OPERATED UNDER THE CCU UMBRELLA ARE:COMPREHENSIVE CARE MANAGEMENT STATE CERTIFIED CARE COORDINATORS VISIT INDIVIDUALS AGE 60 + IN THEIR OWN HOMES, HOSPITALS, NURSING HOMES, ASSISTIVE AND SUPPORTIVE LIVING FACILITIES TO EDUCATE THEM AND THEIR FAMILIES ABOUT THE CHOICES FOR CARE THAT ARE AVAILABLE IN THE COMMUNITY. THEY COMPLETE A NEEDS ASSESSMENT AND DETERMINE ELIGIBILITY FOR HOME AND COMMUNITY BASED SERVICES. ONCE ELIGIBILITY IS DETERMINED THE CARE COORDINATOR AND THE INDIVIDUAL DEVELOP A PERSON CENTERED PLAN OF CARE THAT WILL MEET THE INDIVIDUAL'S NEEDS FROM THE OPTIONS AVAILABLE. THE CARE COORDINATOR MAKES REFERRALS FOR APPROPRIATE PROGRAMS AND THEN MONITORS THE INDIVIDUAL TO MAKE SURE THEIR NEEDS ARE BEING MET. NOT ONLY DO CARE COORDINATORS PREVENT PREMATURE NURSING HOME PLACEMENT THEY ALSO ASSIST ELIGIBLE INDIVIDUALS TO RETURN TO THE COMMUNITY FROM A LONG TERM CARE FACILITY.CHOICES FOR CARE COMPLETES HOSPITAL AND LONG TERM CARE (LTC) FACILITY SCREENINGS PROVIDING CONSULTATIONS FOR 18-60+ INDIVIDUALS THAT NEED TO KNOW OPTIONS FOR PLACEMENT FOR REHAB AND SERVICES WHEN THEY RETURN HOME. ANOTHER SERVICE OFFERED IS THE CCU'S MONEY MANAGEMENT PROGRAM. THIS PROGRAM ASSISTS ELIGIBLE INDIVIDUALS WITH THEIR FINANCES. INSURED AND TRAINED VOLUNTEERS PROVIDE A VARIETY OF SERVICES SUCH AS MAKING SURE MONTHLY BILLS ARE PAID ON TIME, WRITING CHECKS, BALANCING CHECKBOOKS, BUDGETING AND MANAGING MONTHLY GOVERNMENT BENEFITS.THE ADULT SERVICES PROGRAM (APS) STATE CERTIFIED CASE WORKERS RESPOND TO ALLEGATIONS OF ABUSE, NEGLECT AND SELF-NEGLECT AND EXPLOITATION OF VICTIMS AGE 60 + AND INDIVIDUALS AGE 18 TO 59 THAT HAVE A DISABILITY. CASE WORKERS DETERMINE IF THE ALLEGATIONS ARE FOUNDED AND WITH THE VICTIM'S CONSENT DEVELOP A SAFETY PLAN THAT INCLUDES REFERRALS TO LEGAL SERVICES, LAW ENFORCEMENT, ORDERS OF PROTECTION AND OTHER AVAILABLE INTERVENTIONS..OUR INFORMATION AND ASSISTANCE PROGRAM COVERS MANY SERVICES SUCH AS BENEFIT ACCESS (AGING AND DISABLED) ASSISTANCE FOR CAREGIVERS, OPTIONS COUNSELING FOR INDIVIDUALS AND THEIR FAMILIES, ASSIST WITH APPLICATION TO AVAILABLE PROGRAMS, ASSISTANCE WITH MEDICARE ISSUES INCLUDING ENROLLMENT AND APPEALS, AS WELL AS PROVIDING OUTREACH IN THE COMMUNITY MAKING SURE PEOPLE UNDERSTAND THE OPTIONS AVAILABLE TO THEM.ALL CCU PROGRAMS WORK TOGETHER PROVIDING COMPREHENSIVE OPTIONS TO ALL INDIVIDUALS WHO REQUEST THEM.
NUTRITION PROGRAM (SANGAMON & MENARD) SPRINGFIELD SPECIALTY MEALS ON WHEELS DELIVERS LUNCH AND BREAKFAST GENERAL DIET AND DIABETIC MEALS. DAILY BREAD IS OFFERED AT 7 CONGREGATE SITES AND DELIVERY IN MENARD AND SANGAMON COUNTIES. IN ADDITION TO A NOURISHING MEAL, DAILY BREAD ALSO OFFERS PARTICIPANTS AN OPPORTUNITY FOR FELLOWSHIP, INFORMATIVE PROGRAMS AND SOCIAL ACTIVITIES. HOME DELIVERED MEALS ARE AVAILABLE FOR THOSE INDIVIDUALS WHO ARE HOMEBOUND AND ON A GENERAL DIET.AREA OF COVERAGE INCLUDES ATHENS, CHATHAM, DIVERNON, GREENVIEW, PETERSBURG, TALLULA, AND THE SPRINGFIELD SENIOR CENTER.
SENIOR TRANSPORT (SANGAMON COUNTY ONLY) SENIOR TRANSPORT PROVIDES TRANSPORTATION TO PARTICIPANTS, AGE 60 OR OLDER, LIVING IN SANGAMON COUNTY. VEHICLES ARE WHEELCHAIR ACCESSIBLE. POLICIES ARE PROVIDED TO PARTICIPANTS WITH INFORMATION REGARDING PRIORITIES AND DAILY SCHEDULING OF APPOINTMENTS. A PERSONAL INFORMATION FORM (PIF) IS REQUIRED TO BE COMPLETED ON EACH PARTICIPANT BEFORE SERVICE IS PROVIDED. TRANSPORTATION IS PROVIDED TO RURAL COUNTY PARTICIPANTS ON A WEEKLY BASIS AND ENCOMPASSES THE ENTIRE SANGAMON COUNTY AREA.
