INDIVIDUAL SERVICE SUPPORT-MAINTAINS CONTACT WITH PEOPLE IN DISABILITY RELATED PROGRAMS OF THE ILLINOIS DEPARTMENT OF HUMAN SERVICES TO DETERMINE IF SERVICES ARE APPROPRIATE, TO MONITOR INDIVIDUALS PROGRESS, AND TO ENSURE THAT CONSUMERS RIGHTS, HEALTH, INTEREST AND WELFARE ARE SUSTAINED.
DORS TRAUMATIC BRAIN INJURY
CASE COORDINATION-GATHER INFORMATION INCLUDING MEDICAL, PSYCHOLOGICAL, EDUCATIONAL, AND SOCIAL BACKGROUND, AND MAKE SERVICE ARRANGEMENTS FOR INDIVIDUALS WHO REQUIRE ASSISTANCE.
