INDIVIDUAL SERVICE SUPPORT-MAINTAINS CONTACT WITH PEOPLE IN DISABILITY RELATED PROGRAMS OF THE ILLINOIS DEPARTMENT OF HUMAN SERVICES TO DETERMINE IF SERVICES ARE APPROPRIATE, TO MONITOR INDIVIDUALS PROGRESS, AND TO ENSURE THAT CONSUMERS RIGHTS, HEALTH, INTEREST AND WELFARE ARE SUSTAINED.


PRE-ADMISSION SCREENING-COORDINATE ELIGIBILITY DETERMINATION, INDIVIDUAL ASSESSMENTS, SERVICE APPLICATIONS, AND SERVICE ARRANGEMENTS FOR PEOPLE WITH DEVELOPMENTAL DISABILITIES. CSO SERVES AS A CLEARING-HOUSE FOR PERSONS WITH DISABILITIES, AND/OR FAMILY/GUARDIAN, WHO ARE SEEKING SERVICES. IT COMPILES AND DISSEMINATES INFORMATION PERTAINING TO DISABILITY ISSUS, RESOURCES, SERVICES, AND EVENTS; AND ASSERTIVELY EDUCATES FEDERAL, STATE, AND LOCAL POLICY MAKERS ABOUT DISABILITY ISSUES.
