RESIDENTIAL SERVICES IS COMPRISED OF THIRTEEN LOCATIONS: DAVIS HOUSE - 4237 S. INDIANA AVE. (773) 373-1044; MOORE HOUSE - 9135 S. BRANDON AVE. (773) 731-4033; KNIGHT HOUSE - 6600 S. STEWART AVE. (773) 994-0775; HAMMOND HOUSE - 6701 S. MORGAN STREET (773) 994-0833; DANFORTH HOUSE - 4540 S. MICHIGAN AVE. (773) 373-1073; 2659 W. 59TH STREET (ADMINISTRATIVE OFFICE RESIDENTIAL SERVICES (773) 918-6200). WE PROVIDE HOUSING AND REHABILITATION SERVICES FOR ADULTS 16 AND OLDER WHO HAVE DISABILITIES. EMPLOYMENT TRAINING AND MONITORING IS ALSO PROVIDED. TRADITIONAL FOSTER CARE SERVICES (773) 602-2660, FOSTER CARE EMERGENCY SHELTER SERVICES (773)356-6972. WE PROVIDE COLLEGE PREPARATION TOUTORING, SUMMER SCHOOL CLASSES, CULTURAL ENRICHMENT, ACT TEST PREPARATION, AND ASSISTANCE WITH COLLEGE ADMISSION AND FINANCIAL AID SERVICES. PROGRAM SERVED 1,660 STUDENTS THIS YEAR. BEHAVIORAL HEALTH & CLINICAL SERVICES IS LOCATED AT 2715 WEST 63RD STREET (773) 918-6100. WE PROVIDE MENTAL HEALTH CRISIS INTERVENTION, PRE-PSYCHIATRIC HOSPITAL SCREENING ASSESSMENT, IN-HOME AND OFFICER BASED THEERAPEUTIC COUSELING, PSYCHIATRIC/PSYCHOLOGICAL ASSESSMENTS AND MEDICATION MONITORING FOR CHILDREN WITH MENTAL HEALTH AND DEVELOPMENT DISORDERS.
