PROVIDES COORDINATED, COMPREHENSIVE CLINICAL CARE ACROSS HOSPITALS, PHYSICIAN PRACTICES, AND OTHER HEALTHCARE PROVIDERS, WITH AN AIM TO BRING DOWN THE OVERALL COSTS OF MEDICAL CARE. FRANCISCAN ACO, INC. SERVES MEDICARE AND OTHER PATIENTS ACROSS INDIANA AND ILLINOIS THROUGH THE COLLABORATIVE EFFORTS OF THE FOLLOWING PARTNERS: (A) 14 TAX-EXEMPT HOSPITALS OPERATED, DIRECTLY AND INDIRECTLY, BY FRANCISCAN ALLIANCE, INC.; (B) FRANCISCAN PHYSICIAN NETWORK, A FRANCISCAN ALLIANCE, INC.
