ORGANIZATIONS MISSION IS TO ELIMINATE HEALTH CARE DISPARITIES IN AURORA AFRICAN AMERICANCOMMUNITY BY USING A HOLISTIC APPROACH ( PROMOTING PHYSICAL, MENTAL, EMOTIONAL AND SPIRITUAL HEALTH). EACH YEAR IN PARTNERSHIP WITH OTHER COMMUNITY AGENCIES, WE HOST A FREE HEALTH FAIR.

BOOSTING VACCINE AWARENESS FOR ALL-RIMHOSTED VACCINE CLINICS AND COMMUNITY EVENTS IN VARIOUS VENUES FOR RIM POPULATION (RUFUGEES,IMMIGRANTS, MIGRANTS). HOSTED A CULTURALLY AND LINGUISTICALLY TAILORED HEALTH AND RESOURCE FAIR WITH OUR LOCAL COMMUNITY PARTNERS, HEALTHCARE PROVIDERS, AND COMMUNITY LEADERS TO ADDRESS CONCERNS, COMBAT MISINFORMATION, AND IMPROVE VACCINE UPTAKE. ASSISTED THE EVALUATION TEAM WITH DISTRIBUTING, COLLECTING, AND INPUTTING DATA AS NEEDED. DEVELOPED PROGRAM ACTIVITIES AND THE NATIONAL STANDARDS FOR CULTURALLY AND LINGUISTICALLY APPROPRIATE SERVICES TO ENSURE THAT THE PROJECT ACTIVITIES, SERVICES, AND RESOURCES ARE PROVIDED IN AN EQUITABLE MANNER TO OUR HISPANIC AND BLACK RIM POPULATIONS AND HOSTED EDUCATIONAL WORKSHOPS. PROVIDED 1:1 SERVICES, RESOURCES, AND SUPPORT TO THE RIM POPULATION. WORKED WITH OUR COMMUNITY PARTNERS TO IDENTIFY INDIVIDUALS IN NEED OF ADDITIONAL SUPPORT.
UTILIZED THE STATE OF ILLINOIS MESSAGING CAMPAIGN TO PROMOTE CONSISTENT AND INCLUSIVE SAFE SLEEP MESSAGING. INCREASED AWARENESS ABOUT INFANT SAFE SLEEP PRACTICES AND PROVIDE RISK REDUCTION EDUCATION AND TRAINING FOR PARENTS, CAREGIVERS, AND EARLY CHILDHOOD PROFESSIONALS. PROVIDED TRAININGS/EDUCATION INCLUDING , BUT NOT LIMITED TO, MATERNAL STRESS, BREASTFEEDING PROMOTION, SMOKING CESSATION AND SLEEP ENVIRONMENTS. IDENTIFY SOCIAL DETERMINANTS OF HEALTH AND OTHER BARRIERS TO SAFE SLEEP PRACTICES AMONGST FAMILIES IN THE COMMUNITY, AND ADOPT CULTURALLY RELEVANT RESOURCES/APPROACHES TO ADDRESS THESE BARRIERS (E.G., CRIBS, PACK-N-PLAYS, CO-SLEEPERS, SLEEP SACKS).EMPLOYED RISK REDUCTION APPROACHES THAT WILL ACKNOWLEDGE FAMILIES EXISTING PRACTICES AND PROVIDE ADDITIONAL OPPORTUNITIES TO IMPROVE SLEEP ENVIRONMENT AND OTHER SAFE SLEEP PRACTICES.
REACH: REACHING INTO OUR COMMUNITIES TO SUPPORT VACCINE AWARENESSIDENTIFIED POPULATIONS IN AURORA AND ELGIN THAT WERE DISPROPORTIONATELY AFFECTED BY COVID-19. WORKED WITH THE COMMUNITIES TO PROVIDE EDUCATION AND AWARENESS REGARDING THE COVID VACCINE TO INCREASE VACCINE UPTAKE, AND ADDRESS THE GROWING DISPARITY HISPANIC AND BLACK COMMUNITIES. SPECIAL EMPHASIS WAS ON YOUTH AGES 12-18 YEARS. FOCUSED ON POPULATIONS IN OUR TARGETED ZIP CODES THAT HAVE LOW VACCINATION RATES AND PROVIDE EDUCATION ON THE VACCINE. SPECIFICALLY, WE AIMED TO ADDRESS THE BARRIERS AND CHALLENGES AROUND THE VACCINE IN AN EFFORT TO INCREASE VACCINE UPTAKE IN THE BLACK, HISPANIC, AND YOUTH COMMUNITIES LOCATED IN 60123, 60502, 60505,60506
