SUPPORTIVE LIVING PROGRAM PROVIDES WRAP-AROUND SERVICES FOR THOSE WITH MENTAL ILLNESS WHO LIVE INDEPENDENTLY IN THE COMMUNITY. APPROX 200 CLIENTS SERVED.


FOUR ADULT CARE FACILITIES FOR THOSE WITH PERSISTENT RECURRING MENTAL ILLNESS STAFFED 24 HOURS/DAY, 365 DAYS/YR. A TOTAL OF THIRTY-SIX CLIENTS RESIDE IN THESE FOUR FACILITIES.


FOUR PERMANENT SUPPORTIVE HOUSING FACILITIES FOR THE HOMELESS. A TOTAL OF THIRTY-EIGHT FORMERLY HOMELESS CLIENTS WITH MENTAL ILLNESSES RESIDE IN THESE FOUR FACILITIES. THESE FACILITIES ARE STAFFED 24 HOUR/DAY, 365 DAYS/YEAR.
