TRANSPORTATION - PROVIDING TRANSPORTATION FOR MEDICAL PERSONNEL, AND SENIOR FRIENDSHIP AND SOCIAL CENTER ACTIVITIES.
CASE MANAGEMENT - PROVIDE SOCIAL WORKER SERVICES TO ASSESS SENIORS NEEDS AND HELP THEM TO FIND THE SERVICES NEEDED TO KEEP THEM HEALTHY AND TO REMAIN INDEPENDENT IN THEIR HOMES.
NUTRITION - MEALS ON WHEELS PROGRAM DELIVERING HOT MEALS FIVE DAYS A WEEK TO THE AGED AND HOME BOUND INCLUDES CONGREGATE MEALS PROGRAMS PROVIDING ACTIVITIES AND TRIPS.
