VOLUNTEERS PROVIDE TRANSPORTATION, MEAL DELIVERY AND VARIOUS PROGRAMS THAT SERVE CRITICAL NEEDS OF ISOLATED FOOTHILLS NEIGHBORS AGING IN PLACE.

VAN PROGRAM THE ORGANIZATION OPERATES 8 VEHICLES, 7 OF WHICH WERE ACQUIRED WITH PARTIAL FUNDING UNDER THE SECTION 5310 FEDERAL GRANT PROGRAM. FIVE OF THE VEHICLES ARE WHEELCHAIR ACCESSIBLE. EACH VAN IS DRIVEN BY A VOLUNTEER DRIVER AND VOLUNTEER ASSISTANTS ARE ASSIGNED TO TRIPS AS NECESSARY. HOMEBOUND INDIVIDUALS MAY SIGN UP FOR VARIOUS VAN OUTINGS THAT ARE SCHEDULED EACH MONTH. OUTINGS INCLUDE TRIPS TO GROCERY STORES, RESTAURANTS, LIBRARIES, SENIOR EXERCISE CLASSES, EDUCATION COURSES, ETC. THE COVID PANDEMIC SUSPENDED OUTINGS OF THIS PROGRAM FOR MOST OF THE FISCAL YEAR, BUT VANS WERE INSTEAD USED FOR VOLUNTEERS TO VISIT SENIORS AND DISABLED PERSONS IN A SOCIALLY DISTANCED WAY THROUGH OUR SPREAD JOY CONVOYS.
ONE-ON-ONE PROGRAM SERVICES VOLUNTEERS PROVIDE A VARIETY OF SERVICES INCLUDING: HANDYMAN SERVICES-PERFORMING MINOR HOME REPAIRS; SHOPPING SERVICES-PROVIDING TRANSPORTATION AND AN INDIVIDUAL SHOPPING ASSISTANT; FRIENDLY VISITING-PROVIDING REGULAR CONTACT WITH HOMEBOUND INDIVIDUALS BY PHONE OR IN PERSON; CAREGIVING RELIEF-VISITING WITH HOMEBOUND INDIVIDUALS WHILE THE PRIMARY CAREGIVER, USUALLY A RELATIVE, TAKES A BREAK; MOBILITY LOAN CLOSET PROGRAM-MOBILITY DEVICES (WALKERS, WHEELCHAIRS, CANES, ETC.) ARE LOANED TO INDIVIDUALS IN NEED OF SAME; FRIENDLY PET VISITING-BRINGING CERTIFIED PETS TO VISIT HOMEBOUND INDIVIDUALS.
MOBILE MEALS PROGRAM VOLUNTEERS DELIVER UP TO 7 MEALS PER WEEK TO HOMEBOUND QUALIFYING INDIVIDUALS. MEALS ARE DELIVERED BY VOLUNTEERS USING THEIR OWN VEHICLES. FOR THIS FISCAL YEAR, APPROXIMATELY 5,744 MEALS WERE DELIVERED. THE MEALS ARE PREPARED ON A CONTRACT BASIS BY A LOCAL HOSPITAL. ELIGIBLE INDIVIDUALS HAVE THE COST OF THEIR MEALS PROVIDED BY A GRANT FROM THE AREA AGENCY ON AGING REGION ONE, INCORPORATED (PHOENIX, AZ). OTHER INDIVIDUALS PAY FOR THEIR MEALS AT A PRICE OF $7.25 PER MEAL. FOR THE FISCAL YEAR ENDED JUNE 30, 2021, PRIVATE PAY INDIVIDUALS PAID $15,726 AND THE ARIZONA AREA ON AGING FUNDED $29,403.
MOBILE MEALS AND WELLNESS CHECK PROGRAM - VOLUNTEERS PERFORM WELLNESS CHECKS AND DELIVER NUTRITIOUS HOT AND COLD MEALS PREPARED BY HONOR HEALTH THOMPSON PEAK HOSPITAL TO COMMUNITY MEMBERS ("FCC NEIGHBORS"), THOSE WHO ARE HOMEBOUND AND HAVE QUALIFYING NEEDS SUCH AS DISABLING CONDITIONS, SURGERY RECOVERY, ILLNESS, SUDDEN INJURY, MOBILITY CHALLENGES OR DIFFICULTY IN FOOD PREPARATION. ALL MEALS MEET AREA AGENCY ON AGING REGION ONE, INCORPORATED (PHOENIX, AZ) DIETARY GUIDELINES FOR THE AGING POPULATION AND ARE LOW IN SALT AND SUGAR TO HELP THOSE FOLLOWING A MODIFIED DIET. FINANCIALLY ELIGIBLE FCC NEIGHBORS HAVE THE COST OF THEIR MEALS PROVIDED BY A GRANT FROM THE AREA AGENCY ON AGING REGION ONE, INCORPORATED (PHOENIX, AZ). OTHER FCC NEIGHBORS PAY FOR THEIR MEALS AT A PRICE OF $7.25 PER MEAL. DURING FISCAL YEAR 2021/2022, THE FCC VOLUNTEERS FAITHFULLY CARRIED OUT ALL REQUIRED IN HOME WELLNESS CHECKS ENSURING 185 NEIGHBOR'S SAFETY IN THEIR HOMES WHILE AGING IN PLACE AND SUCCESSFULLY DELIVERED APPROXIMATELY 26,210 NUTRITIOUS AND DELICIOUS MEALS.
TRANSPORTATION PROGRAMS - 1. NON-EMERGENCY MEDICAL TRANSPORTATION: VOLUNTEERS DRIVE THEIR OWN VEHICLES TO TRANSPORT AND PERSONALLY ACCOMPANY FCC NEIGHBORS TO AND/OR FROM DIALYSIS, CANCER TREATMENT, PHYSICAL THERAPY, AND OTHER MEDICAL AND DENTAL APPOINTMENTS MAKING SURE THAT EACH NEIGHBOR ARRIVES BACK HOME SAFE AND SOUND. 2. WHEEL SAFE: WITH ADDITIONAL PASS TRAINING, CERTIFIED FCC VOLUNTEERS DRIVE OR MAY SERVE AS AN ASSISTANT IN ONE OF THE 5 WHEELCHAIR ACCESSIBLE VANS TO SAFELY TRANSPORT AND TRANSFER FCC NEIGHBORS WHO HAVE SPECIAL WHEELCHAIR ACCESSIBLE TRANSPORTATION NEEDS TO AND FROM NON-EMERGENCY MEDICAL APPOINTMENTS. 3. RECREATIONAL VAN PROGRAM: FCC VOLUNTEERS DRIVE AND ACCOMPANY NEIGHBORS IN GROUPS TO SHOP AT GROCERY STORES AND LOCAL FOOD BANK, AS WELL AS TRANSPORT AND SUPPORT THEM AT SCHEDULED SOCIAL OUTINGS, INCLUDING FITNESS/EXERCISE CLASSES, EDUCATIONAL PROGRAMS, WELLNESS AND A WIDE VARIETY OF COMMUNITY EVENTS TO HELP FIGHT SOCIAL ISOLATION AND PROMOTE A SENSE OF COMMUNITY. FCC OPERATES AND IS RESPONSIBLE FOR THE MAINTENANCE AND SAFETY OF A FLEET OF 8 VAN VEHICLES, 7 OF WHICH WERE ACQUIRED WITH PARTIAL FUNDING UNDER THE SECTION 5310 FEDERAL GRANT PROGRAM. FIVE OF THE VEHICLES ARE WHEELCHAIR ACCESSIBLE. EACH VAN IS DRIVEN BY A VOLUNTEER DRIVER WITH VOLUNTEER ASSISTANTS AS NECESSARY.
