LUTHERAN SENIORLIFE PASSAVANT COMMUNITY IS A 501(C) (3) LIFE PLAN COMMUNITY WITH A 115+ YEAR HISTORY OF PROVIDING FAITH-BASED, NON-PROFIT SERVICES TO ELDERS IN WESTERN PENNSYLVANIA. LOCATED ON 42 ACRES IN ZELIENOPLE, PA, PASSAVANT COMMUNITY IS ONE OF LUTHERAN SENIORLIFE'S SENIOR LIVING COMMUNITIES DEDICATED TO PROVIDING OPPORTUNITIES FOR ABUNDANT LIFE TO ELDERS. WE PROVIDE A COMPLETE CONTINUUM OF CARE INCLUDING RESIDENTIAL LIVING IN COTTAGES, VILLAS, AND APARTMENTS, PERSONAL CARE IN TWO VENUES, LONG TERM NURSING, SHORT TERM REHABILITATION, MEMORY CARE, AND ADULT DAY CARE. OUR ABUNDANT LIFE CENTER ACCOMMODATES 102 ELDERS EITHER REQUIRING SHORT TERM SKILLED REHABILITATION SERVICES OR NEEDING LONG TERM NURSING SERVICES. THE NURSING COMMUNITY IS DESIGNED IN A HOUSEHOLD MODEL WITH 6 NURSING HOUSEHOLDS. EACH HOUSEHOLD HAS ACCOMMODATIONS FOR 17 RESIDENTS. THIS LIFE AFFIRMING ENVIRONMENT AFFORDS RESIDENTS PRIVACY IN 96 PRIVATE ROOMS AND THREE (3) SEMI-PRIVATE SUITES. EACH HOUSEHOLD IS ALSO APPOINTED WITH A FULL SERVICE KITCHEN AND DINING ROOM ALONG WITH A FAMILY ROOM AND ACCESS TO THE OUTDOORS. WE CONTINUE OUR WORK TO TRANSFORM OUR CULTURE TO ONE THAT IS TRULY PERSON-DIRECTED, WHERE RESIDENTS DIRECT THE COURSE OF THEIR DAY. EACH ELDER'S VOICE IS HONORED AND WE CONTINUALLY EVALUATE OUR PRACTICES AND ADAPT OUR PROCESSES TO BETTER MEET THEIR NEEDS. PASSAVANT COMMUNITY PROVIDES SERVICE TO PERSONAL CARE RESIDENTS IN TWO DIFFERENT VENUES IN NEWHAVEN COURT AND IN THE ABUNDANT LIFE CENTER. NEWHAVEN COURT HAS 92 APARTMENTS BUILT IN A NEIGHBORHOOD MODEL AND THE ABUNDANT LIFE CENTER HAS TWO HOUSEHOLDS, ONE OF WHICH IS SPECIFICALLY DESIGNED FOR INDIVIDUALS NEEDING MEMORY SUPPORT. WE OFFER VARIED OPTIONS FOR INDIVIDUALS WANTING TO LIVE RESIDENTIALLY FROM SEVERAL DIFFERENT APARTMENT BUILDINGS TO VARIED COTTAGE STYLES. THE ABUNDANT LIFE CENTER IS THE COMMUNITY'S HUB AND PROVIDES VARIED COMMUNITY LIFE OFFERINGS FROM THE FITNESS CENTER TO VARIED DINING VENUES. MANY OF THE RESIDENTS ARE ACTIVE VOLUNTEERS BOTH ON THE CAMPUS AS WELL AS IN THE COMMUNITY AT LARGE. THEY TEACH CLASSES, FACILITATE PROGRAMS, AND PROVIDE VALUABLE INPUT IN OPERATIONAL DECISIONS THAT AFFECT THEM DIRECTLY. WE CONTINUE TO OFFER OUR ADULT DAY PROGRAM KNOWN AS MY DAY OUT TO BOTH RESIDENTS OF OUR CAMPUS AS WELL AS THE SURROUNDING COMMUNITY AND WE CONTINUE TO BE AN ALTERNATIVE CARE SITE FOR LUTHERAN SENIORLIFE'S LIFE BUTLER PROGRAM AS WELL AS A PROVIDER THROUGH THE AREA AGENCY ON AGING. THE MEDICAL CLINIC ON CAMPUS SERVES RESIDENTS THROUGH PRIMARY CARE PHYSICIANS AND SPECIALISTS. GUIDED BY ITS FAITH-BASED NON-PROFIT MISSION, RESIDENTS ACCEPTED INTO PASSAVANT COMMUNITY'S CONTINUUM OF CARE ARE NOT ASKED TO LEAVE SHOULD THEY OUTLIVE THEIR RESOURCES. IN FY 2021, PASSAVANT COMMUNITY PROVIDED SUPPORT OF $793,234 THROUGH ITS BENEVOLENT CARE PROGRAM.
