HOSPICE & COMMUNITY CARE PROVIDES PERSONALIZED CARE AND COMFORT TO HELP PATIENTS AND FAMILIES LIVE BETTER WITH SERIOUS ILLNESS THROUGH END OF LIFE. DURING THE CURRENT YEAR, HOSPICE & COMMUNITY CARE PROVIDED OVER 188,000 PATIENT DAYS OF HOSPICE CARE TO PATIENTS AND THEIR FAMILIES AND SERVED 3,945 PATIENTS DURING THE YEAR, REGARDLESS OF THE PATIENT'S ABILITY TO PAY. ALMOST FOUR PERCENT OF THE PATIENT DAYS OF HOSPICE CARE WERE CHARITABLE CARE. OVER 32,000 DAYS OF INTERDISCIPLINARY SUPPORTIVE CARE WERE PROVIDED TO 341 PATIENTS AND THEIR FAMILIES, REGARDLESS OF THE PATIENT'S ABILITY TO PAY. PHYSICIANS COMPLETED OVER 12,000 HOSPICE AND PALLIATIVE CARE VISITS TO PATIENTS IN THEIR PRIVATE HOMES, NURSING HOMES, HOSPITALS, PHYSICIAN CLINIC AND HOSPICE INPATIENT UNIT. THE BEREAVEMENT PROGRAM SUPPORTED 8,888 INDIVIDUALS DURING THE CURRENT YEAR THROUGH 177 SUPPORT GROUPS, ALMOST 43,000 PIECES OF MAIL, OVER 3,600 TELEPHONE CALLS, AND 853 INDIVIDUAL VISITS.
