PROVIDE ASSESSMENT AND CASE MANAGEMENT SERVICES UNDER THE DEPARTMENT OF VETERANS' AFFAIRS VETERAN-DIRECTED HOME AND COMMUNITY BASED SERVICES PROGRAM. SERVICES ARE PROVIDED THROUGH CONTRACTS WITH THE FIVE VERMONT AREA AGENCIES ON AGING AND OTHER ORGANIZATIONS.
PROVIDE SUPPORT TO THE FIVE VERMONT AREA AGENCIES ON AGING. SUPPORT INCLUDES COMPUTER TECH DEVELOPMENT, PUBLIC AWARENESS, AND ADMINISTRATION OF STATEWIDE CONTRACTS.
