RHI MAKES HOME VISITS TO FARM FAMILIES WHERE OUTREACH HEALTH COORDINATORS PERFORM HEALTH RISK ASSESSMENTS, BLOOD PRESSURE AND CHOLESTEROL SCREENINGS, AND MAKES REFERRALS FOR HEALTH CARE PROVIDERS AND SOCIAL SERVICES AT NO COST TO THE FARM FAMILIES.

TO PROVIDE REGULAR VISITS TO FARM FAMILIES IN SHAWANO, MARATHON, WAUPACA AND OUTAGMIE COUNTIES. WITH EACH VISIT, HEALTH RISK ASSESSMENTS ARE PERFORMED, INCLUDING NUMEROUS BLOOD PRESSURE AND CHOLESTEROL SCREENINGS WHICH CAN LEAD TO MANY REFERRALS TO HEALTH AND SOCIAL SERVICES. IN ADDITION TO THIS, EDUCATION IS PROVIDED TO THOSE FARM FAMILIES IN NEED.
