D/B/A THE HARRY AND JEANETTE WEINBERG CAMPUS ("WEINBERG CAMPUS") IS AN EXEMPT ORGANIZATION AND IS THE SOLE MEMBER FOR 8 EXEMPT OPERATING SUBSIDIARIES INCLUDING MENORAH CAMPUS INDEPENDENT SENIOR APARTMENTS, INC. WEINBERG CAMPUS, THROUGH ITS OPERATING SUBSIDIARIES, OFFERS A VARIETY OF LIVING OPTIONS AND CUSTOMIZABLE CARE PROGRAMS TO MEET THE NEEDS OF SENIORS IN WESTERN NEW YORK. THE SERVICES PROVIDED RANGE FROM INDEPENDENT LIVING APARTMENTS WITH SUPPORTIVE SERVICES AVAILABLE AS NEEDED TO A MORE STRUCTURED ADULT HOME SETTING, TO SOCIAL AND MEDICAL DAY PROGRAMS, SHORT TERM REHABILITATION, A NURSING HOME, HOME CARE, AND A MENTAL HEALTH RESIDENCE. WEINBERG CAMPUS IS A LEADER IN DEVELOPING THOUGHTFUL APPROACHES TO THE CHALLENGES OF AGING AND STRIVES TO CREATE A HOME-LIKE ENVIRONMENT THAT ASSISTS INDIVIDUALS TO LIVE ACCORDING TO THEIR NEEDS AND VALUES. WEINBERG CAMPUS PROVIDES SERVICES TO APPROXIMATELY 2,000 INDIVIDUALS ON AN ANNUAL BASIS. MENORAH CAMPUS INDEPENDENT SENIOR APARTMENTS ("THE APARTMENTS") PROVIDE 455 APARTMENTS FOR SENIORS IN THE WESTERN NEW YORK COMMUNITY. THE APARTMENTS ARE PROVIDED IN FIVE DISTINCTIVE SETTINGS: (1) 52 APARTMENTS FOR SENIORS WHO WISH TO ENJOY A RELATIVELY INDEPENDENT LIFESTYLE IN BEAUTIFUL APARTMENTS WITH TAILORED SERVICES DESIGNED TO MEET THEIR NEEDS; (2) 64 APARTMENTS DESIGNED FOR ADULTS WHO WISH TO AGE IN A PLACE WITHOUT CONCERNS OF HOUSEKEEPING, PROPERTY MANAGEMENT, AND MEAL PREPARATION; (3) 96 APARTMENTS TO SUPPORT THE FULL RANGE OF ISSUES THAT ACCOMPANIES ELDERLY FRAIL; (4) 120 APARTMENTS FOR ADULTS WHO PARTICIPATE IN MANAGED LONE-TERM CARE PROGRAM; AND (5) A 3-STORY, 123-UNIT SENIOR APARTMENT BUILDING DESIGNED TO HOUSE INDIVIDUALS 55 AND OLDER IN AN ACTIVE AND VIBRANT SETTING. FEATURES INCLUDE: 24-HOUR SECURITY, ACTIVITIES PROGRAM THAT PROVIDES GROUP AND INDIVIDUAL ATTENTION FOR RESIDENTS TO ENJOY, ACCESS TO ALL CAMPUS AMENITIES SUCH AS THE GIFT SHOP, DELI, BANKING SERVICES, COMPUTER ROOM, BARBER SHOP AND SALON, AND TRANSPORTATION TO SHOPPING COMMUNITY ACTIVITIES AND OFF-CAMPUS VISITS.
