PROVIDED HOSPICE CARE AND SUPPORT FOR TERMINALLY ILL PATIENTS AND THEIR FAMILIES IN A HOME SETTING (10,559 PATIENT DAYS) AND IN A HOSPICE RESIDENCE (2,495 PATIENT DAYS). PROIVDED PALLIATIVE CARE, MEDICAL CARE FOR PEOPLE LIVING WITH A SERIOUS ILLNESS, TO THOSE IN THE COMMUNITY. PALLIATIVE CARE IS FOCUSED ON PROVIDING RELIEF FROM THE SYMPTOMS AND STRESS OF THE ILLNESS TO IMPROVE QUALITY OF LIFE FOR BOTH THE PATIENT AND THE FAMILY.
