THE MEALS ON WHEELS PROGRAM PROVIDES DELICIOUS AND NUTRITIOUS HOT AND COLD MEALS, PREPARED IN OUR INDUSTRIAL KITCHEN LOCATED IN THE SOUTH END OF ALBANY. ALL OF OUR MEALS ARE MADE FROM SCRATCH AND PROVIDE EACH RECIPIENT WITH 1/3 TO 2/3 OF THE DAILY RECOMMENDED NUTRITIONAL REQUIREMENTS. THIS PROGRAM IS MORE THAN A MEAL AS EACH DELIVERY ALSO INCLUDES A FRIENDLY WELLNESS CHECK. WE SERVE OVER 350 OLDER ADULTS DAILY.
SENIOR CENTER ENRICHMENT - PROGRAMS AND ACTIVITIES INCLUDING RECREATION, SOCIALIZATION AND EDUCATIONAL OPPORTUNITIES FOR OLDER ADULTS THROUGHOUT OUR REGION. MORE THAN 1500 OLDER NEIGHBORS AND FRIENDS PARTICIPATE IN AT LEAST ONE OF OUR MANY PROGRAMS AND ACTIVITIES. CONGREGATE DINING IS AVAILABLE MONDAY THROUGH FRIDAY AT VARIOUS SITES THROUGHOUT ALBANY COUNTY WHICH INCLUDE THE TOWN OF BERNE COMMUNITY AND SENIOR SERVICES CENTER, OHAV SHOLOM APARTMENTS, SOUTH MALL SENIOR CENTER, LIVINGSTON AND WESTVIEW HOMES. A HOT, NUTRITIOUS MEAL SERVED IN AN ENJOYABLE ATMOSPHERE, OFTEN AUGMENTED WITH ENTERTAINMENT, IS THE IDEAL WAY TO SOCIALIZE AND MINGLE WITH FRIENDS. OVER 13,900 MEALS WERE SERVED IN 2021.
NEWGATE IS A SOCIAL ADULT DAY PROGRAM THAT PROVIDES SUPERVISION, SOCIALIZATION AND STIMULATION TO OLDER ADULTS WHO MAY REQUIRE EXTRA SUPPORT AND/OR COMPANIONSHIP DURING THE DAY AND RESPITE TO THE OVERWHELMED CAREGIVER. STAFFED BY CARING PROFESSIONALS WE CAN SERVE 11-20 SENIORS A DAY WHEN NOT RESTRICTED DUE TO COVID.
HIICAP - HEALTH INSURANCE INFORMATION AND COUNSELING TO MORE THAN 1,500 SENIORS AND TRAINING TO 15+ PROFESSIONALS ANNUALLY.HEALTH AND WELFARE PROGRAMS - EDUCATION AND STRATEGIES FOR MAINTAINING OPTIMUM HEALTH. CLASSES SUCH AS TAI CHI AND CHAIR YOGA, AS WELL AS EVIDENCE BASED SEMINARS INCLUDING AGING MASTERY PROGRAM, FALLS TALK, LIVING WITH DIABETES AND PATIENT ACTIVATION MEASUREMENT (PAM). PROGRAM PROVIDES ASSESSMENT, COACHING AND SUPPORT TO IMPROVE THE OVERALL HEALTH AND WELLNESS OF OLDER INDIVIDUALS. COMMUNITY CASE MANAGEMENT - ONE ON ONE CASE MANAGEMENT AND CARE COORDINATION FOR A VARIETY OF LIFE TRANSITIONS SUCH AS: HOUSING OPTIONS, RETIREMENT, HEALTH AND WELLNESS, AND RESOURCES TO REMAIN INDEPENDENT AND IMPROVE ONE'S QUALITY OF LIFE.THE CAREGIVER CONNECTION - EDUCATIONAL OPPORTUNITIES AND INTENSIVE PERSONALIZED COUNSELING TO CAREGIVERS AT HOME OR IN THE WORKPLACE, INCLUDING THE TELEPHONE SUPPORT GROUP WHICH PROVIDES CAREGIVERS OF OLDER ADULTS WITH INFORMATION, RESOURCES, AND A VITAL CONNECTION TO OTHER CAREGIVERS.
