HOME AND COMMUNITY BASED SERVICES - OPERATES 14 INDIVIDUALIZED RESIDENTIAL ALTERNATIVES FACILITIES (IRA), WHICH PROVIDES SUPPORTIVE SERVICES TO 76 DEVELOPMENTALLY DISABLED INDIVIDUALS. WE ALSO OPERATE 5 INDIVIDUALIZED SUPPORTED SERVICES (ISS), WHICH PROVIDES RESIDENTIAL OPPORTUNITIES FOR 5 DEVELOPMENTALLY DISABLED INDIVIDUALS. THESE RESIDENCES ARE LOCATED IN THE BRONX BROOKLYN AND QUEENS PROVIDING A WIDE RANGE OF RESIDENTIAL AND CLINICAL SERVICES. IN ADDITION, WE PROVIDE COMMUNITY AND DAY HABILITATION WITHOUT WALLS VOCATIONAL INTEGRATED SERVICES WITHIN THE COMMUNITY TO THESE INDIVIDUALS. THROUGH OUR EXTENSIVE NETWORK OF AFFILIATIONS ADDITIONAL OFF-SITE SERVICES ARE PROVIDED BASED ON EACH INDIVIDUAL'S SPECIFIC NEEDS; THESE INCLUDE PSYCHIATRIC, PHYSICAL THERAPY, OCCUPATIONAL THERAPY, SPEECH AND
COMMUNITY RESIDENCES - OPERATES 3 RESIDENTIAL FACILITIES WHICH PROVIDES SUPPORTIVE SERVICES TO 140 FORMERLY HOMELESS MENTALY ILL INDIVIDUALS AND 5 SUPPORTED HOUSING PROGRAMS FOR UP TO 103 FORMERLY HOMELESS INDV. WHO SUFFER FROM MENTAL ILLNESS. THE RESIDENTIAL PROGRAMS ARE COMMUNITY RESIDENCES SINGLE ROOM OCCUPANCY (CR/SRO) WHICH OFFER EXTENDED-STAY HOUSING TO INDV. WHO MEET THE CRITERIA ESTABLISHED BY NYS OFFICE OF MENTAL HEALTH. ALL INDIVIDUALS REFERRED MUST HAVE BEEN RECENTLY HOMELESS AND SUFFER FROM A SERIOUS AND PERSISTENT MENTAL ILLNESS.
