OUR MISSION IS TO ENHANCE THE WELL-BEING OF INDIVIDUALS AND FAMILIES THROUGH THE HUMAN SERVICES DELIVERY SYSTEM(S), WITH PARTICULAR CONCENTRATION ON THOSE INDIVIDUALS THAT HAVE TRADITIONALLY EXPERIENCED DIFFICULTY IN ACCESSING HUMAN SERVICES. THEY INCLUDE (WITHOUT LIMITATION) PERSONS WITH DISABILITIES, DISADVANTAGED, LATINO, AFRICAN AMERICAN, NATIVE AMERICAN AND/OR MIGRANT. FOR OVER THREE DECADES SINERGIA HAS ASSISTED UNDER-SERVED FAMILIES, MANY WITH LIMITED ENGLISH PROFICIENCY, AFRICAN AMERICAN, FAMILIES FACING HOMELESSNESS AND FAMILIES EXPERIENCING POVERTY.

COMMUNITY RESIDENTIAL SERVICES - THE ORGANIZATION OPERATES A VARIETY OF COMMUNITY INTEGRATED RESIDENTIAL FACILITIES UNDER LICENSE FROM THE NYS OFFICE FOR PEOPLE WITH DEVELOPMENTAL DISABILITIES (OPWDD), WITH ONSITE RESIDENTIAL HABILITATION STAFF SUPPORT THAT WORK WITH RESIDENTS TO ACHIEVE COMMUNITY INCLUSION, INTEGRATION, INDEPENDENCE, AND PRODUCTIVITY. THESE SERVICES ARE DEVELOPED PREMISED ON PERSON CENTERED PRINCIPLES AND GOALS.
COMMUNITY SERVICES - COMPRISED OF CATEGORIES OF MEDICAID WAIVER FEE FOR SERVICE ACTIVITIES INVOLVING THE DELIVERY OF SUPPORTS IN NON-CERTIFIED HOME AND COMMUNITY SETTINGS. THESE INCLUDE IN-HOME RESPITE, IN-HOME RESIDENTIAL HABILITATION AND COMMUNITY HABILITATION.
DAY HABILITATION AND THE SUPPLEMENTAL UP LIVING PROGRAM ENGAGES INDIVIDUALS WITH DEVELOPMENTAL DISABILITIES IN A COMPREHENSIVE DAY PROGRAM THAT DEVELOPS THEIR EDUCATIONAL, SOCIAL, RECREATIONAL AND VOCATIONAL SKILLS.
