THE MENTAL HEALTH ASSOCIATION OF NASSAU COUNTY IS A NOT-FOR-PROFIT MEMBERSHIP ORGANIZATION DEDICATED TO IMPROVING MENTAL HEALTH THROUGH ADVOCACY, EDUCATION, PROGRAM DEVELOPMENT AND THE DELIVERY OF DIRECT SERVICES. AS WE CELEBRATE 66 YEARS OF SERVICE, OUR VISION IS TO OPEN DOORS AND INSPIRE HOPE. WE PROVIDE CONCRETE SERVICES TO THE MOST VULNERABLE POPULATIONS BY IMPACTING SYSTEMATIC CHANGES AND UTILIZING GRASS ROOTS EFFORTS TO PROMOTE OUR MESSAGE OF HOPE TO LIVE THEIR MOST MEANINGFUL LIFE.

RESIDENTIAL SERVICES: OFFICE OF MENTAL HEALTH/OFFICE OF HOUSING AND URBAN DEVELOPMENT:COMMITMENT TO PROVIDING THE CONCRETE ELEMENTS OF LIFE: SAFE AND STABLE HOUSING, EMPLOYMENT, MONEY MANAGEMENT, CARE COORDINATION, PEER SUPPORT, AS WELL AS, HOME AND COMMUNITY BASED SERVICES (HCBS). OUR SUPPORTIVE SERVICES ARE PROVIDED TO ASSIST INDIVIDUALS MOVE THROUGH THE CONTINUUM OF CARE, TO ACHIEVE INDEPENDENCE AND BECOME VALUED MEMBERS OF THEIR COMMUNITIES. RESIDENTIAL SERVICES INCLUDE: COMMUNITY RESIDENCES, APARTMENT TREATMENT PROGRAM AND SUPPORTIVE HOUSING. OUR IMPACT IS IMPRESSIVE, SERVING 125 IN 2021. IN ADDITION TURQUOISE HOUSE IS A CRISIS RESPITE/HOSPITAL DIVERSION PEER SUPPORT PROGRAM THAT IS COMPRISED OF A RESPITE HOUSE AND WARM LINE. SERVED 157 UNDUPLICATED INDIVIDUALS AND DIVERTED 95 HOSPITALIZATIONS.


CARE COORDINATION PROGRAMS: HEALTH HOMES: PROVIDES A WIDE-VARIETY OF SUPPORT AND SERVICES IN IN NON-INSTITUTIONAL SETTINGS FOR ADULTS AND CHILDREN WITH MENTAL HEALTH AND SUBSTANCE ABUSE CHALLENGES UTILIZING A PERSON-CENTERED APPROACH TO ENSURE WHOLE PERSON WELLNESS. SERVICES ALSO INCLUDE ASSISTANCE WITH TRAVEL, FINANCIAL ENTITLEMENTS, AND EDUCATIONAL AND VOCATIONAL NEEDS. SUPPORT FOR AT RISK INDIVIDUALS TO HELP NAVIGATE THE MENTAL HEALTH SYSTEM AND LINK THEM TO NECESSARY SERVICES AND SUPPORT IN THE AREAS OF HEALTH, FINANCIAL, ENTITLEMENTS, EDUCATIONAL AND VOCATIONAL NEEDS. MEDICAID SERVICE COORDINATION/CRISIS RESPITE: OUTREACH AND REFERRALS FOR ADULTS AND CHILDREN WHO HAVE CHALLENGES ACCESSING RESOURCES, BENEFITS, SUPPORT SERVICES, HOUSING, AND EDUCATIONAL OPPORTUNITIES. DURING 2021, CARE COORDINATION CONTINUALLY PROVIDED SERVICES TO OVER 503 INDIVIDUALS.


PERSONALIZED RECOVERY ORIENTED SERVICES (PROS): PROS IS A COMPREHENSIVE RECOVERY ORIENTED PROGRAM FOR INDIVIDUALS WITH MENTAL HEALTH CHALLENGES ACTIVELY PARTICIPATING IN SKILL DEVELOPMENT AND IMPROVEMENT IN OVERALL HEALTH WITH GOAL OF BEING AN ACTIVE PARTICIPANT IN THEIR COMMUNITIES. PROGRAMS INCLUDE WELLNESS MANAGEMENT, TRAVEL TRAINING, FINANCIAL LITERACY, EDUCATION AND VOCATIONAL TRAINING, AND CREATIVE ARTS THERAPY. SUPPORT GROUPS ARE OFFERED TO INDIVIDUALS AND FAMILIES. PROS PROVIDES CLINICAL TREATMENT SERVICES DESIGNED TO HELP STABILIZE AND DEVELOP WAYS TO MANAGE SYMPTOMS. CLINICAL TREATMENT PROVIDES A RECOVERY FOCUSED, DISABILITY MANAGEMENT APPROACH WITH MEDICATION MANAGEMENT, HEALTH ASSESSMENT, CLINICAL COUNSELING AND THERAPY, SYMPTOM MONITORING, AND TREATMENT FOR CO-OCCURRING DISORDERS, ALL INTEGRATED WITH PROS REHABILITATIVE SERVICES TO PROVIDE COMPREHENSIVE CARE. MHA OF NASSAU COUNTY, INC. PROVIDED 11,221 HOURS OF DIRECT CARE SERVICES FOR PROS PROGRAMS DURING 2021.


OTHER MHA PROGRAMS: FINANCIAL MANAGEMENT: SUPPORTS INDIVIDUALS IN MANAGING THEIR FINANCES TO ENSURE HOUSING STABILITY AND APPROPRIATE USE OF PERSONAL FUNDS. FINANCIAL LITERACY EDUCATION AND BUDGET MANAGEMENT IS PROVIDED TO PROMOTE INDEPENDENT SKILLS FOR MONEY MANAGEMENT. FINANCIAL MANAGEMENT SERVED 536 INDIVIDUALS IN 2021. VETERAN'S HEALTH ALLIANCE OF LONG ISLAND (VHALI) AND VET2VET PROGRAMS PROVIDE OUTREACH, ADVOCACY, SUPPORT AND TRAINING TO VETERANS AND THEIR FAMILIES. IN 2021, THEY SUPPORTED 4,500 VETERANS. ADVOCACY, EDUCATION & TRAINING, A PARTNER WITH THE DEPARTMENT OF MENTAL HEALTH, CHEMICAL DEPENDENCY AND DEVELOPMENTAL DISABILITIES, PROVIDES COMMUNITY BASED PSYCHO-EDUCATIONAL TRAININGS FOR BEHAVIORAL HEALTH PROFESSIONALS AND ADVOCATES ON BEHALF OF THE MENTAL HEALTH SYSTEM TO RAISE MENTAL HEALTH AWARENESS AND ENHANCE EDUCATION IN THESE AREAS TO THE COMMUNITY AT LARGE. PROVIDED WORKSHOPS AND PROFESSIONAL TRAINING TO APPROXIMATELY 2,500 INDIVIDUALS IN 2021. CONSUMER LINK, PEER RUN DIVISION, PROVIDES PEER SUPPORT AND MOBILE OUTREACH TO ALL DEPARTMENTS WITHIN THE ORGANIZATION AND COMMUNITY. IN 2021 MHANC NY PROJECT HOPE IS A CRISIS COUNSELING PROGRAM OF THE NYS OFFICE OF MENTALHEALTH THAT PROVIDES INDIVIDUAL ASSESSMENTS, EMOTIONAL SUPPORT, PUBLIC EDUCATION, AND RESOURCES TO HELP OUR COMMUNITIES COPE WITH THE CHALLENGES OF COVID-19.
