COMMUNITY LIVING ARRANGEMENTS: COMMUNITY GROUP HOMES THAT SUPPORT ADULTS WITH INTELLECTUAL DISABILITIES TO LIVE AND PARTICIPATE IN THEIR COMMUNITY. INDIVIDUALS RECEIVE SUPPORT TO REALIZE THEIR PERSONAL POTENTIAL AND PURSUE INDEPENDENT, PRODUCTIVE AND MEANINGFUL LIVES.
INDIVIDUAL & FAMILY SUPPORT: THIS SERVICE AREA PROVIDES RESIDENTIAL, CLINICAL, AND VOCATIONAL SUPPORT TO INDIVIDUALS WITH INTELLECTUAL DISABILITIES, DEVELOPMENTAL DISABILITIES, AND AUTISM. SUPPORT IS PROVIDED TO INDIVIDUALS AND FAMILIES ACROSS THE LIFESPAN. SERVICES ARE PROVIDED IN INDIVIDUAL APARTMENTS, LICENSED COMMUNITY COMPANION HOMES, AND FAMILY HOMES. INDIVIDUALS ARE SUPPORTED TO ACHIEVE THEIR INDEPENDENCE, MAKE APPROPRIATE PERSONAL CHOICES, ACHIEVE GOALS RELATED TO WORK, COMMUNITY PARTICIPATION, AND RETIREMENT ACTIVITIES.
NURSING SERVICES: COMPREHENSIVE NURSING SERVICES ADDRESS THE NEEDS OF ADULTS AND CHILDREN WITH INTELLECTUAL DISABILITIES, DEVELOPMENTAL DISABILITIES, AND AUTISM. NURSES PROVIDE ASSESSMENT, ROUTINE SUPPORT, COORDINATION OF CARE, AND WRITTEN HEALTH REPORTS. NURSING SERVICES INCLUDES RN ON CALL, WHICH MAKES A REGISTERED NURSE AVAILABLE BY PHONE 24 HOURS A DAY.
