TO PROVIDE A FULL RANGE OF INNOVATIVE HOSPICE, BEREAVEMENT, PALLIATIVE CARE, AND OTHER SUPPORT AND IMPROVE THE QUALITY OF LIFE FOR A PATIENT WITH A LIFE-LIMITING ILLNESS AND THEIR CAREGIVERS IN THE SETTING OF THEIR CHOICE.

THE CORPORATION PROVIDES MEDICALLY NECESSARY SERVICES TO PATIENTS WHO MEET CERTAIN CRITERIA UNDER ITS HOSPICE AND PALLIATIVE CARE ADMISSION CRITERIA. THE CORPORATION ALSO OFFERS SUPPORT TO ITS PATIENTS AND THEIR FAMILY WITH ANY EMOTIONAL, SOCIAL, AND SPIRITUAL NEEDS THEY MAY HAVE. SERVICES ARE PROVIDED TO PATIENTS LIVING IN THE HOSPICE CENTER, INDIVIDUAL RESIDENCES, NURSING HOMES, AND ASSISTED LIVING FACILITIES.
