TRANSITION HOUSE IS A HUMAN SERVICES AGENCY DEDICATED TO ENDING AND PREVENTING DOMESTIC VIOLENCE.

HOUSING PROGRAMS: TRANSITION HOUSE PROVIDES AN EMERGENCY SHELTER: A CONGREGATE SHELTER IN A RESIDENTIAL HOME-LIKE SETTING THAT PROVIDES SAFE, EMERGENCY HOUSING AND HIGHLY SKILLED 24/7 TRAUMA SUPPORT FOR SURVIVORS OF DOMESTIC VIOLENCE AND THEIR CHILDREN. SUPPORT SERVICES INCLUDE: COUNSELING, SAFETY PLANNING, SKILL BUILDING WORKSHOPS, HEALTHCARE ACCESS AND ON-SITE VISITING HEALTH CARE, HOUSING SEARCH, CHILDCARE ADVOCACY AND PERIODIC ON-SITE CHILD CARE. OUR CONTINUUM OF HOUSING RESOURCES ALSO INCLUDES: TRANSITIONAL HOUSING AND SUPPORT SERVICES FOR 9-24 MONTHS IN SCATTERED SITE, COMMUNITY-BASED UNITS SHARED BY SURVIVORS OF DOMESTIC VIOLENCE AND THEIR CHILDREN TRANSITIONING FROM EMERGENCY SHELTER TO SAFE, AFFORDABLE HOUSING AND PERMANENT SUPPORTED HOUSING FOR SURVIVORS WITH DISABILITIES. SUPPORT SERVICES INCLUDE: COUNSELING, CASE MANAGEMENT, SKILL BUILDING RESOURCES, HOUSING SEARCH AND ACCESS TO AN ARRAY OF NEARBY COMMUNITY SERVICES. IN FY23, THESE HOUSING PROGRAMS SERVED 85 ADULTS AND CHILDREN, AND FIELDED OVER 665 HOTLINE CALLS.


COMMUNITY BASED SERVICES: PATHWAYS TO SAFETY & PATHWAYS TO PERMANENT HOUSING: TRANSITION HOUSE PROVIDES HOLISTIC SUPPORT, COUNSELING SERVICES, AND INTERVENTION RESOURCES FOR SURVIVORS OF DOMESTIC VIOLENCE THROUGHOUT OUR DIVERSE COMMUNITY; INCLUDING PARTNER ABUSE, DATING VIOLENCE AND FAMILY VIOLENCE. IN COLLABORATION WITH KEY COMMUNITY PARTNERS, WE PROVIDE ADVOCACY AND COUNSELING SUPPORT, SERVING AS AN INTERVENTION AND RESOURCE HUB FOR PEOPLE OF ALL AGES, BACKGROUNDS, AND INCOME LEVELS TO PREVENT HOMELESSNESS. OTHER SERVICES INCLUDE HOUSING ADVOCACY, LEGAL ADVOCACY, COURT ACCOMPANIMENT, SUPPORT GROUPS, PUBLIC BENEFITS ADVOCACY AND REFERRALS TO A WIDE RANGE OF RESOURCES. WE ALSO DELIVER CUSTOM TRAINING ON DOMESTIC VIOLENCE INTERVENTION AND RESPONSE TO PROFESSIONALS IN LAW ENFORCEMENT, SOCIAL SERVICES, EDUCATION, ELDER SERVICES AND PROPERTY MANAGEMENT. IN FY23, THIS PROGRAM PROVIDED DIRECT SERVICES TO OVER 87 PEOPLE AND PROVIDED TRAINING FOR 314 PEOPLE.
