HOME CARE SERVICES OFFERS CARE MANAGEMENT AND A RANGE OF IN HOME AND COMMUNITY BASED SERVICES FOR ELDERS WHO ARE LOW INCOME AND FUNCTIONALLY IMPAIRED SO THEY CAN AGE IN PLACE AT HOME AND AVOID OR DELAY NURSING HOME CARE. ALSO PROVIDES SUPPORT FOR FAMILY CAREGIVERS.
COMMUNITY SUPPORT OFFERS FREE INFORMATION ON HOME AND COMMUNITY BASED RESOURCES FOR SENIORS, DISABLED ADULTS AND CAREGIVERS TO PROMOTE INDEPENDENT LIVING AND HEALTHY AGING.
