COMMUNITY CHOICES IS A MEDICAID FUNDED PROGRAM DESIGNED TO PROVIDE HOME CARE SUPPORT FOR ELDERS WHO ARE ELIGIBLE FOR MASS HEALTH COVERAGE AND AT RISK OF PLACEMENT IN A LONG-TERM FACILITY. THE PROGRAM PROVIDED SERVICES TO 6,960 PUBLICLY SPONSORED CLIENTS IN FY2022.
OTHER PROGRAMS PROVIDE CLINICAL ASSESSMENT OF ELDERS, SUPPORTIVE SENIOR HOUSING, MONEY MANAGEMENT, GERIATRIC MENTAL HEALTH SERVICES, HEALTH INSURANCE INFORMATION, MEALS AND NUTRITION SERVICES, CAREGIVER SUPPORT, ELDER TRANSPORTATION, AND LONG TERM CARE OPTIONS COUNSELING.
