OUR MISSION IS TO IMPROVE THE HEALTH OF THE PEOPLE THROUGHOUT THE BERKSHIRES BY DELIVERING EXCEPTIONAL, COMPASSIONATE HEALTH CARE AND FAMILY SERVICES.

COMMUNITY HEALTH PROGRAMS, INC. (CHP) IS A FEDERALLY QUALIFIED HEALTH CENTER WITH COMPREHENSIVE MEDICAL PRACTICES WHICH ARE THE ONLY PRIMARY CARE PROVIDERS IN BERKSHIRE COUNTY THAT ARE ACCESSIBLE TO ALL PEOPLE, REGARDLESS OF INSURANCE STATUS, IMMIGRANT STATUS, OR ABILITY TO PAY. CHP PROVIDES PREVENTATIVE, ACUTE, AND ONGOING MEDICAL CARE FOR INFANTS, CHILDREN, ADULTS, AND ELDERS. CHP PROVIDES FULL, 24 HOUR MEDICAL COVERAGE FOR OUR PATIENTS, 365 DAYS A YEAR. MEMBERS OF OUR PROVIDER TEAM OF PHYSICIANS AND NURSE PRACTITIONERS ARE CERTIFIED IN INTERNAL MEDICINE, OBSTETRICS & GYNECOLOGY, PEDIATRICS, CLINICAL SOCIAL AND BEHAVIORAL HEALTH, AND FAMILY MEDICINE. EACH PATIENT CHOOSES ONE OF THESE PROVIDERS TO BE THEIR PRIMARY CARE PROVIDER AND THE ENTIRE STAFF IS AVAILABLE TO MEET THE MANY HEALTHCARE NEEDS OF PATIENTS. THE MEDICAL PRACTICES SERVED OVER 92,000 PATIENT VISITS DURING FY2022.


COMMUNITY HEALTH PROGRAMS, INC. (CHP) HAS THREE COMPREHENSIVE DENTAL PRACTICES THAT ARE ACCESSIBLE TO ALL PEOPLE, REGARDLESS OF INSURANCE STATUS, IMMIGRANT STATUS, OR ABILITY TO PAY. THE DENTAL PRACTICES OFFER A GROUP OF DENTAL HEALTH PROFESSIONALS DELIVERING HIGH QUALITY DENTAL CARE USING THE LATEST DIGITAL IMAGING TECHNOLOGY. THESE DENTAL SERVICES INCLUDE: DENTAL CONSULTATIONS, DENTAL X-RAYS, PREVENTATIVE DENTISTRY, DENTAL CLEANINGS, SEALANTS, FLUORIDE APPLICATIONS, RESTORATIVE PROCEDURES, ROOT CANALS, PROSTHETICS INCLUDING CROWNS, PARTIALS, AND DENTURES, EMERGENCY DENTAL CARE, DENTAL TRAUMA MANAGEMENT, PATIENT AND PARENT DENTAL EDUCATION, AND REFERRALS FOR SPECIALTY DENTAL CARE. THE DENTAL CENTERS SERVED OVER 16,000 PATIENT VISITS DURING FY2022.


OTHER HEALTH RELATED SERVICES INCLUDE THE FAMILY SERVICES / WIC (WOMEN INFANT AND CHILDREN) PROGRAMS LOCATED IN SOUTHERN BERKSHIRE AND SERVICING ALL OF BERKSHIRE COUNTY WITH PROGRAMS SUCH AS PCHP (PARENT CHILD HOME PROGRAMS), LACTATION CONSULTING, CLOTHING EXCHANGE AND LENDING LIBRARY, PLAYGROUPS, PARENT WORKSHOPS, FOOD PANTRY AND MOBILE FOOD BANKS, AND CHILDREN'S ATTIC.
