HOME CARE IS AN INTEGRAL COMPONENT OF THE LONG TERM CARE CONTINUUM, PROVIDING QUALITY, COST-EFFECTIVE HEALTH CARE SERVICES NOT ONLY IN INDIVIDUALS' PRIVATE HOMES, BUT IN ASSISTED LIVING, NURSING HOMES, AND IN RETIREMENT COMMUNITIES. "HOME CARE" REFERS TO A WIDE RANGE OF HEALTH AND SOCIAL SERVICES, INCLUDING MEDICAL, NURSING, SOCIAL, OR THERAPEUTIC TREATMENT AND/OR ASSISTANCE WITH THE ESSENTIAL ACTIVITIES OF DAILY LIVING. CAREGIVER SERVICES PROVIDED THROUGH OUR HOME CARE AGENCY GENERALLY INCLUDE PERSONAL CARE, COMPANIONSHIP AND SUPERVISION, AS WELL AS HELP IN THE HOME WITH THE ACTIVITIES OF DAILY LIVING (ADL) AND THE INSTRUMENTAL ACTIVITIES OF DAILY LIVING (IADL) SUCH AS MEAL PREPARATION, MEDICATION REMINDERS, LAUNDRY, LIGHT HOUSEKEEPING, ERRANDS, SHOPPING, TRANSPORTATION, AND COMPANIONSHIP. CABRINI CARE AT HOME (CCAH) WAS A NYS LICENSED HOME CARE SERVICES AGENCIES (LHCSA), THAT PROVIDED HOME CARE SERVICES TO CLIENTS WHO LIVE IN THE FIVE BOROUGHS OF NEW YORK CITY AND WESTCHESTER COUNTY.AS A LICENSED HOME CARE SERVICES AGENCY, CABRINI CARE AT HOME OFFERED HOME CARE SERVICES TO CLIENTS WHO PAID PRIVATELY OR HAD INSURANCE COVERAGE. CCAH ALSO CONTRACTED TO PROVIDE SERVICES TO MEDICARE/MEDICAID BENEFICIARIES WHOSE CASES WERE MANAGED BY A CERTIFIED HOME HEALTH AGENCY OR HOSPICE PROGRAM OR A MANAGED HEALTH PLAN. DURING 2019, CCAH OFFICIALLY DISSOLVED WITH THE NEW YORK STATE.
