HEALTH CARE LA IPA'S (HCLA) MISSION IS TO PROVIDE COMMUNITY-BASED PROVIDERS IN LOS ANGELES COUNTY WITH A MANAGED, INTEGRATED HEALTHCARE DELIVERY SYSTEM TO SERVE THEIR COMMUNITIES IN AN ORGANIZED, EFFICIENT, COMPASSIONATE AND FINANCIALLY RESPONSIBLE MANNER.

HCLA OPERATES AN INDEPENDENT PRACTICE ASSOCIATION MADE UP OF NON-PROFIT HEALTH CENTERS THAT PROVIDE MEDICAL SERVICES, ESPECIALLY TO LOW INCOME AND MEDICALLY UNDERSERVED POPULATIONS IN LOS ANGELES COUNTY. HCLA CONTRACTS WITH OVER 38 FEDERALLY QUALIFIED HEALTH CENTERS (FQHCS) AND COMMUNITY CLINICS THROUGHOUT LOS ANGELES COUNTY TO PROVIDE COORDINATED HEALTHCARE TO THE UNDERSERVED POPULATION IN THEIR COMMUNITIES, INCLUDING: (1) POST DISCHARGE CARE COORDINATION ON THE FRAIL ELDERLY PATIENTS. CASE MANAGERS ARE ASSIGNED TO PATIENTS WITH COPD, CHF, AND OTHER CHRONIC DISEASE CONDITIONS. (2) IN HOME HEALTH ASSESSMENT EXAMS TO SENIOR PATIENTS WHO HAVE NOT BEEN SEEN BY A PRIMARY PROVIDER WITHIN SIX MONTHS OF ENROLLMENT. A NURSE PRACTITIONER PERFORMS A HEALTH ASSESSMENT AND ROUTINE LABS. RESULTS ARE SHARED WITH PRIMARY PHYSICIAN FOR FOLLOW UP VISIT. (3) PROGRAMS THAT PROVIDE ADDITIONAL ASSISTANCE TO THE COMMUNITY CLINICS, SUCH AS PROVIDING HEALTH CENTERS WITH THE NECESSARY TRAINED PERSONNEL TO ASSIST WITH MEETING THE TRIPLE AIM OF THE AFFORDABLE CARE ACT: THE RIGHT CARE, AT THE RIGHT SETTING, FOR THE MOST AFFORDABLE COST; AND TO SERVE AS THE PRINCIPAL APPLICANT AND RECIPIENT OF GRANTS AND OTHER FUNDS FOR PROGRAMS THAT REQUIRE THE PARTICIPATION OF TWO OR MORE OF THE NONPROFIT HEALTH CENTERS.
