THE ORGANIZATION PROVIDED 14,387 PATIENT DAYS OF CARE, FOR 254 INDIVIDUALS AFFECTED BY ALZHEIMER'S DISEASE OR ANOTHER DEMENTIA, INCLUDING MEDICAL, THERAPEUTIC, NUTRITIONAL, AND PERSONAL CARE THROUGH TEMPORARY ALTERNATIVE SERVICE. 6,490 TRIPS BETWEEN PATIENT HOMES AND THE CENTER WITH OUR DOOR-TO-DOOR TRANSPORTATION SERVICE. 10,160 HOURS OF INTENSIVE CARE MANAGEMENT BY SOCIAL WORKERS AND NURSES FOR 254 UNDUPLICATED PATIENTS AND 635 CAREGIVERS OF PATIENTS WITH ALZHEIMER'S DISEASE OR ANOTHER DEMENTIA. 1,772 NURSING INTERVENTIONS TO NOTIFY PHYSICIANS AND FAMILIES OF A CHANGE IN PATIENT'S CONDITION AND COORDINATE MEDICAL INTERVENTIONS. 23,387 MEALS SERVED TO THE ORGANIZATION'S PATIENTS THAT MEET OR EXCEED NUTRITIONAL STANDARDS FOR GERIATRIC PATIENTS. 1,891 HOURS OF BEHAVIORAL HEALTH SERVICES PROVIDED TO PATIENTS AND CAREGIVERS THROUGH THE OUTPATIENT SERVICES BEHAVIORAL HEALTH PROGRAM.
