THE ORGANIZATION PROVIDED 13,503 PATIENT DAYS OF CARE, FOR 150 INDIVIDUALS AFFECTED BY ALZHEIMER'S DISEASE OR ANOTHER DEMENTIA, INCLUDING MEDICAL, THERAPEUTIC, NUTRITIONAL, AND PERSONAL CARE THROUGH TEMPORARY ALTERNATIVE SERVICES. 6,000 HOURS OF INTENSIVE CARE MANAGEMENT BY SOCIAL WORKERS AND NURSES FOR 150 UNDUPLICATED PATIENTS AND 375 CAREGIVERS OF PATIENTS WITH ALZHEIMER'S DISEASE OR ANOTHER DEMENTIA. 2,200 NURSING INTERVENTIONS TO NOTIFY PHYSICIANS AND FAMILIES OF A CHANGE IN PATIENT'S CONDITION AND COORDINATE MEDICAL INTERVENTIONS. 1,765 HOURS OF BEHAVIORAL HEALTH SERVICES PROVIDED TO PATIENTS AND CAREGIVERS THROUGH THE OUTPATIENT SERVICES BEHAVIORAL HEALTH PROGRAM. OVER 650 ATTENDEES TO CAREGIVER SUPPORT GROUPS ANNUALLY AT THE ORGANIZATION.
